目的探讨肝硬化门静脉高压性牌动脉高压(portopulmonary hypertension, POPH)的临床特点及免疫学改变。方法将100例肝硬化患者分为POPH组及非POPH组,比较两组患者的临床特点。测定POPH患者T细 胞亚群及NKCD16+CD56细胞的表达水平。结果100例肝硬化并门静脉高压患者中10例合并POPH。POPH大小与Child-Pugh分级无关(P>0.05)。两组患者丙肝肝硬化患病率差异有统计学意义(P<0.05)。两组间患者合并(portal vein thrombosis, PVT)的比例差异有统计学意义(50% vs 16.7% ,P<0.05)。8例病毒性肝硬化并 POPH患者中,有6例患者NKCD16+CD56细胞数目降低。结论门脉高压肝硬化患者可伴发POPH。HCV感染、PVT以及NKCD16+CD56降低在POPH形成过程中可能起重要作用。 |

Objective To review the clinical characteristics and immunological changes of patients with portopulmonary hypertension (POPH).Methods One hundred patients with liver cirrhosis were divided into POPH and the non-POPH groups. The clinical characteristics were compared between two groups, and T cell and NKCD16+56 cell levels in peripheral blood samples were measured in POPH patients. Results There were 10 patients with POPH in 100 cirrhosis patients. POPH was not correlated with Child-Pugh classes (P>0.05).However, there were significant differences in prevalence of HCV cirrhosis and portal vein thrombosis (PVT) between two groups (P<0.05). NKCD16 +56 cells were decreased in 6 of 8 viral liver cirrhosis patients with POPH. Conclusion Patients with portal hypertension liver cirrhosis may complicated by POPH,and HCV infection,PVT,decreased NKCD16+56 cells may be associated with POPH. |
