目的 探讨部分免疫活性细胞与自身免疫性溶血性贫血(autoimmune hemolytic anemia,AIHA)发病的关系及输血影响。方法 将67例AIHA患者作为观察组,其中28例输血患者为A组,70例非AIHA其他贫血患者作为对照组,其中30例输血患者为B组。采用流式细胞术测定观察组部分免疫细胞活性水平,并分别与对照组、正常健康组测定值比较,同时对A、B两组患者进行输血前后部分免疫指标比较。结果 ①观察组用药前外周血CD3+、CD4+、CD4+/CD8+、NK细胞活性水平均显著低于正常组(P<0.05);观察组CD19~+指标上升明显高于对照组,而对照组CD19+指标又明显高于正常组,(P<0.05)均有统计学意义。②联合检测AIHA患者T细胞亚群、B细胞、NK细胞的阳性表达率高于单项阳性表达率。③A组输血后外周血CD3+、CD4+、CD4+/CD8+比值上升、CD19+细胞表达下降较治疗前有明显差异。结论 AIHA患者存在外周血部分免疫活性细胞表达异常,输血能改善AIHA患者的细胞免疫平衡调节功能。 |

Abstract: |
Objective To investigate the cellular immunity autoimmune hemolytic anemia(AIHA) and the effect of transfusion.Methods Sixty seven AIHA patients,including 28 receiving blood transfusion (group A) and 70 non-AIHA patients,including 30 receiving transfusion(group B) were enrolled in the study.The levels of CD3+,CD4+,CD4+/CD8+,CD19+ were determined by flow cytometry(PCM). Those immune indicators were compared between ALHA and non-AIHA patients;before and after transfusion.Results The levels of the peripheral blood of T cell subgroup and NK cell activities in AIHA patients were significantly lower than those of non-AIHA group(P<0.05).The abnormal rate of combined tests of T cell subgroup,B cells and NK cells in AIHA patients was higher than that of single tests. The expression of CD3+,CD4+,and CD4+CD8+ in group A increased and CD19+ declined after transfusion. Conclusion Blood transfusion can improve the abnormal cell immunity function in AMA patients. |
