| 摘要: |
| 目的 评价应用上胸段硬膜外阻滞（thoracic epidural blockade,TEB）治疗缺血性心肌病顽固性心力衰竭的疗效。方法 选择缺血性心肌病顽固性心力衰竭患者40例随机分成两组,每组20例,对照组使用常规内科治疗方法,观察组在内科治疗基础上加用TEB,治疗2周后比较心功能分级、心脏指数、LVEF、BNP、心率及血压。结果 两组治疗后均有改善心功能,TEB组心功能改善更明显（P＜0.05）。结论 缺血性心肌病顽固性心力衰竭患者应用TEB治疗有助于改善心功能和预后,提高患者生活质量。 |

| Objective To evaluate the application of high thoracic epidural blockade(TEB) in treatment of refractory heart failure in patients with ischemic cardiomyopathy.Methods Forty ischemic cardiomyopathy patients with refractory heart failure were randomly divided into two groups with 20 in each;the control group received conventional treatment,the TEB group received ropivacaine TEB in addition of conventional treatment.After 14 days of treatment the NYHA Grading cardiac function,cardiac index,LVEF,BNP,hear rate and blood pressure were compared between two groups.Results The cardiac functions in both groups were improved after treatment;however, the improvement in TEB group was more effective than that in control group(P＜0.05).Conclusion Ropivacaine epidural anesthesia can effectively improve cardiac function and quality of life for patients with ischemic cardiomyopathy. |
