能量代谢测定在ICU危重患者中的研究进展

摘要:
营养支持治疗是重症监护室(intensive care unit, ICU)危重症患者综合治疗中不可或缺的一部分,但是营养支持不足或过度喂养均可导致代谢并发症的发生,影响患者预后。合理的营养支持以危重症患者的静息能量消耗(resting energy expenditure, REE)为前提,但ICU患者病情复杂多变,能量代谢紊乱,因此,精准的能量代谢测定能够指导更为合理的支持方案。能量代谢测定的方式包括直接测定、间接测定及公式预测法等,每一种方法在临床应用均有其优缺点。本文就能量代谢测定在ICU危重症患者中的进展进行综述。

Abstract:
Nutritional support therapy is an indispensable part of comprehensive treatment for critical patients in ICU, but insufficient nutritional support or overfeeding may lead to metabolic complications and affect the prognosis of patients. Reasonable nutritional support is based on the actual energy metabolism of critical patients, however, the condition of ICU patients is complex and changeable, and the energy metabolism disorder often occurs. Therefore, the accurate determination of energy metabolism can guide a more reasonable support scheme. The methods of energy metabolism measurement include direct calorimetry, indirect calorimetry and formula prediction. Each method has its advantages and disadvantages in clinical application. In this article, the research progress on energy metabolism of critically ill patients in ICU is reviewed.
