妊娠期营养管理对孕妇体质量控制及妊娠结局的影响

摘要:
目的 探讨孕期个体化营养指导对孕妇体质量控制及妊娠结局的影响。方法 该研究为前瞻性随机对照研究,按照随机数字表将自2011年1月至2012年12月期间在上海交通大学医学院附属国际和平妇幼保健院正规产检的单胎孕妇176例随机分为研究组(79例)和对照组(97例);研究组产妇在孕期由专业营养师进行1~4次个体化营养指导,对照组仅按常规进行产检。结果 对照组和研究组研究对象的年龄、孕前体质量58.91±7.47kg vs 56.45±6.10kg和孕前体质量指数22.02±2.43 vs 21.63±2.41无显著性差异;对照组和研究组平均体质量增加分别为13.58±5.28kg和13.99±3.41kg,(P=0.528);研究组孕前体质量指数低于对照组,但差异不显著,前者孕期体质量指数的增加5.34±1.25高于后者5.07±1.97,差异显著(P=0.001);两组对象的妊娠高血压疾病、妊娠期糖尿病(GDM)及剖宫产率无显著差异,而巨大儿(14.4% vs 5.1%)、足月低出生体质量儿(5.2% vs 0%)及早产(13.4% vs 3.8%)的比例对照组高于研究组,差异显著。结论 孕期给予个体化的营养指导对改善妊娠结局有临床意义。

Abstract:
Objective To evaluate the effects of nutrition guidance during pregnancy on maternal weight control and pregnancy outcomes. Methods This prospective randomized controlled study with cluster sampling in singleton pregnant women was conducted in International Peace Maternity & Child Health Hospital Affiliated to Shanghai Jiaotong University School of Medicine. A total of 176 pregnant women were recruited and divided into study group(79 cases) and control group(97 cases) according to the random number table. Individual nutrition instructions were given by nutritionists to pregnant women in study group for 1 to 4 times, while routine prenatal care was given to women in control group only. Results There was no significant difference in age, pre-pregnancy weight 58.91±7.47kg vs 56.45±6.10kg and pre-pregnancy BMI 22.02±2.43 vs 21.63±2.41 between control group and study group. The mean weight gain were 13.58±5.28kg(control group) and 13.99±3.41kg(study group), respectively(P=0.528). In study group, the BMI gain were increased significantly 5.34±1.25 vs 5.07±1.97(P=0.001). There was no significant difference in hypertensive disorders complicating pregnancy, GDM and rate of cesarean section between two groups; whereas the incidence rates of fetal macrosomia(14.4% vs 5.1%), low birth weight(5.2% vs 0%), and preterm labor(13.4% vs 3.8%)were significantly higher in control group than those in study group. Conclusion Individual nutrition guidance during pregnancy can improve pregnancy outcomes.
