右美托咪定复合瑞芬太尼用于颈丛阻滞下甲状腺手术的效果观察

摘要:
目的 观察右美托咪定(Dexmedetomidine, DEX)复合瑞芬太尼(Remifentanil)用于颈丛阻滞下甲状腺手术的效果。方法选取2012年2月至2013年9月在我院行择期行甲状腺手术患者60例,随机分为右美托咪定组(D组,20例)、右美托咪定复合瑞芬太尼组(DR组,20例)、瑞芬太尼组(R组,20例),分别应用相应药物。记录并比较各时间段患者的平均动脉压(MAP)、心率(HR)、血氧饱和度及Ramsay镇静评分等指标。结果 三组患者术前MAP与HR无显著差异(P>0.05);手术开始后三组患者MAP、HR均较术前逐渐增加(P<0.05),在切除肿块时达到最大值;三组患者手术各时间点循环指标比较,R组患者MAP、HR值均最高,D组次之,DR组最低(P<0.05)。术中D组、DR组Ramsay评分优于R组(P<0.05)。结论右美托咪定复合瑞芬太尼用于颈丛阻滞下甲状腺手术,其镇静效果好,人体应激反应小,安全有效,值得临床推广。

Abstract:
Objective To evaluate the application of dexmedetomidine combined with remifentanil in cervical plexus block anesthesia for thyroid surgery. Methods Sixty patients scheduled for elective thyroid surgery under cervical plexus block anesthesia were randomly assigned to receive dexmedetomidine (D group), remifentanil (R group) or dexmedetomidine combined with remifentanil (DR group) with 20 cases in each groups. The mean arterial pressure (MAP), heart rate (HR), partial pressure of oxygen (SpO2) and Ramsay sedation score were documented and compared among three groups. Results There were no significant differences in preoperative MAP and HR among three groups (P>0.05), while MAP and HR were elevated when the surgery started (P<0.05). R group had the highest and DR group had the lowest MAP and HR during surgery and the Ramsay sedation score in R group was lower than that in D and DR group(P<0.05). Conclusion Dexmedetomidine combined with remifentanil has the best effect in thyroid surgery patients with cervical plexus block anesthesia.
