可塑性纤维桩修复上颌前牙漏斗状根管的实验研究

摘要:
目的 比较不同桩核修复离体前牙漏斗状根管的效果。方法对30颗上前牙完善根充后,在唇侧釉牙骨质界(cemento enamel junction, CEJ)上方2mm处截冠,随机分为A、B、C组。A组用镍镉金属铸造桩核,B组用RelyXTM Fiber纤维桩核,C组用EverStick可塑性纤维桩核,3组均用铸造冠完成修复。所有样本均用自凝树脂包埋,加载机的加载头与牙长轴的夹角为135°,加载频率为150N/s,至牙体折裂,记录其最大抗折负荷以及各组样本的折裂方式,测量牙体折裂最低点的位置。结果 3组间最大负荷差异有统计学意义(P<0.05),可塑性纤维桩核抗折强度最高;3组间折裂点至CEJ距离差异有统计学意义(P<0.05);B组及C组桩冠再次修复的可能性显著高于A组,差异有统计学意义(P<0.05),而B组及C组可再次修复性之间的差异无统计学意义(P>0.05)。结论可塑性纤维桩修复的漏斗状根管的前牙具有较高的抗折强度,并具有良好的二次修复性。

Abstract:
Objective To compare the efficacy of different post-core systems in repairing of maxillary labial teeth with funnel-shaped root canals. MethodsThirty extracted maxillary labial teeth were fully root canal filled and each crown was sectioned at 2mm above the labial cement enamel junction(CEJ). The teeth were randomly divided into three groups: group A, teeth restored with Ni-Cr cast metal post; group B, with RelyXTM fiber post; group C, with EverStick fiber post. Three groups were all restored with cast metal crown finally. All samples were embedded in acrylic resin blocks, then subjected to a compressive load at 150N/s crosshead frequency delivered at a 135° to the long axis. The loading was ceased until the first sign of failure was observed. The maximal fracture load and the fracture mode were recorded. Meanwhile, the lowest places of fracture points were also recorded. ResultsMaximal fracture resistances of three groups were significantly different(P<0.05) and EverStick fiber post indicated the highest fracture resistance. The differences of the distances from CEJ to fracture points were statistically significant among three groups(P<0.05). Both groups B and C showed significantly higher possibility of secondary restoration than group A. Meanwhile, there was no significant difference in secondary restoration between group B and group C(P>0.05). ConclusionMaxillary labial teeth with funnel-shaped root canals indicate higher resistance to fracture and excellent secondary restoration after treated with EverStick fiber post.
