血清尿酸水平和红细胞分布宽度与老年慢性心力衰竭的关系

摘要:
目的探讨血清尿酸(uric acid,UA)水平与红细胞分布宽度(red blood cell distribution width,RDW)与慢性心力衰竭(chronic heart failure,CHF)的关系。方法选择CHF患者276例作为CHF组,NYHAⅡ级88例,Ⅲ级106例,Ⅳ级患者有82例。选择同期健康体检者180例作为对照组。检测并比较两组患者血清UA水平和RDW,同时比较CHF组显效、有效、无效患者的血清UA水平和RDW。结果治疗前,CHF组血清UA水平和RDW均明显高于对照组,随心功能分级的增加,CHF组患者血清UA水平和RDW逐渐升高(P〈0.05)。治疗后,心功能Ⅱ、Ⅲ、Ⅳ级患者血清UA水平和RDW均明显下降(P〈0.05),且心功能Ⅱ、Ⅲ、Ⅳ级患者血清UA水平和RDW比较,差异有统计学意义(P〈0.05)。治疗显效和有效的患者血清UA水平和RDW均下降(P〈0.05),治疗无效的患者血清UA水平和RDW无明显变化(P〉0.05)。结论血清UA水平和RDW可以判断CHF患者的治疗效果极预后。

Abstract:
Objective To investigate the serum uric acid( UA) level and erythrocyte distribution width( RDW) in patients with chronic heart failure( CHF). Methods Twohundred and seventy-six patients with CHF( CHF group),including88 cases with NYHA grade Ⅱ,106 with grade Ⅲ and 82 with grade Ⅳ,and 180 healthy subjects( control group) were enrolled in the study. Level of serum UA and RDW in twogroups were tested and compared. Results Level of serum UA and RDW in CHF group were significantly higher than those in control group. Serum UA and RDW in CHF group were increased with the increase of cardiac function classification before treatment( P〈0. 05). After treatment serum UA level and RDW were declined significantly in CHF patients( P〈0. 05),while there were significant differences amongpatients with NYHA grade Ⅱ,Ⅲ,Ⅳ. Serum UA level and RDW were significantly declined in markedly effective and effective CHF patients,while were not changed in ineffective patients. Conclusion Level of serum UA and RDW are helpful for the assessment of therapeutic efficacy and prognosis of patients with CHF.
