重度COPD患者BMI与肺功能的关系

摘要:
目的研究重度慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者体质量指数(body mass index,BMI)与肺功能之间的相关性。方法选取2011年1月至2012年12月确诊的重度COPD稳定期男性患者89例,按照BMI分成3组:低BMI组(BMI≤18.4 kg/m2)17例,正常BMI组(BMI 18.5~24.99 kg/m2)48例,高BMI组(BMI〉25 kg/m2)24例。3组分别进行肺功能测定,参数比较采用独立样本t检验,相关分析采用Pearson相关性检验。结果低BMI组的FEV1[(0.80±0.15)L]、FEV1%pred[(30.50±4.78)%]、FEV1/FVC[(47.32±8.81)%]低于高BM I组[(0.97±0.23)L]、[(34.05±5.08)%]、[(56.39±8.51)%],差异有统计学意义(P〈0.01,P〈0.05,P〈0.01)。正常BM I组的FEV1[(0.85±0.18)L]、FEV1%pred[(30.50±6.21)%]、FEV1/FVC[(50.83±11.03)%]低于高BM I组[(0.97±0.23)L]、[(34.05±5.08)%]、[(56.39±8.51)%],差异有统计学意义(P〈0.01,P〈0.05,P〈0.05)。85例重度COPD患者的BM I与FEV1、FEV1%pred、FEV1/FVC呈正相关(r=0.321,P〈0.01;r=0.219,P〈0.05;r=0.322,P〈0.01)。结论重度COPD患者BMI与肺功能有显著相关性。

Abstract:
Objective To investigate the correlation between body mass index( BMI) and lungfunction in patients with severe chronic obstructive pulmonary disease( COPD). Methods Eighty nine patients with severe stable COPD were admitted in out hospital from January 2011 toOctober2013. Amongall patients,17 cases had lowbody mass index( BM I≤18. 4 kg/ m2),48 had normal BM I(18. 5-24. 99 kg/ m2) and 24 had high BMI( 25 kg/m2). The pulmonary function test was performed and compared with t test and correlation was analyzed with Pearson correlation test. Results The FEV1,FEV1 / FVC and FEV1% pred in low,normal and high BM I groups were(0. 80 ± 0. 15)L,(0. 85 ±0. 18)L,(0. 97 ± 0. 23)L,(47. 32 ± 8. 81)%,(50. 83 ± 11. 0)3%,(56. 39 ± 8. 51)%; and(30. 50 ±4. 78)%,(30. 50 ± 6. 21)%,(34. 05 ± 5. 08)%,respectively. The FEV1,FEV1 /FVC and FEV1% pred in lowBM I group were significantly lower than those in high BM I groups(P〈0. 01 or P〈0. 05). The FEV1,FEV1 / FVC and FEV1% pred in normal BM I group were significantly lower than those in high BM I groups( P〈0. 01 or P〈0. 05). In patients with severe COPD,the BM I was positively correlated with FEV1(r = 0. 321,P〈0. 01),FEV1% pred(r = 0. 219,P〈0. 05) and FEV1 /FVC( r = 0. 322,P〈0. 01). Conclusion The body mass index is positively correlated with lungfunction in patients with severe COPD.
