预防性中央区淋巴结清扫对乳头状甲状腺癌术后复发影响的meta分析

摘要:
目的评价预防性中央区淋巴结清扫对乳头状甲状腺癌术后复发的影响。方法通过搜索中外数据库获得符合入选标准的6篇文献,采用随机效应模型对文献进行meta分析,计算相对危险度。结果共6个临床研究1 315例符合入选条件,其中632例进行了甲状腺全切除术加预防性中央区淋巴结清扫术,683例单行甲状腺全切术。清扫组84例复发,采样组62例复发。与单行甲状腺全切组相比,联合清扫手术组术后复发发生率差异无统计学意义(P〉0.05)。结论预防性中央区淋巴结清扫对乳头状甲状腺癌术后复发影响不明显。

Abstract:
Objective To assess the effect of prophylactic central neck dissection( p CND) on recurrence of local papillary thyroid carcinoma( PTC) after thyroidectomy. Methods Clinical studies met the inclusion criteria were obtained by literature searching in various medicine databases. The meta-analysis was performed by usinga random effects model. The relative risk was calculated.Results Six clinical studies involving1 315 patients were obtained for analysis,including632 patients treated with thyroidectomy and p CND( p CND group) and 683 patients treated with thyroidectomy alone( control group). The overall recurrence was 84 in p CND group versus 62 in control group.There was nosignificant difference in local recurrence rate between twogroups( P〉0. 05).Conclusion The meta-analysis indicates that p CND may not reduce local recurrence rate after thyroidectomy.
