目的探讨2型糖尿病患者CA199的变化特点及相关影响因素。方法收集180例住院2型糖尿病患 者,并按10)八1<:水平分为血糖控制差组(八组,56例,1?)八|<:>7.5%)、控制尚可组(8组,59例,HbA,C6.5% ~ 7. 5% )、控制良好组(C组,65例.HbAiC <6.5% ),选取同期60例健康人群作为正常对照组(D组)。检测各组血 清CA199、空腹血糖(FBG)、糖化血红蛋白(HbAf)、空腹腠岛素(FINS)、空腹CJt(FCP)年龄、性别、体质量指数 (BMI)、腰围、总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇等指标。比较各组CA199的水平, 并分析与上述各变量间的相关性。以toCA199为因变量,其余指标为自变量,进行多元线性回归分析,比较各变量 对其影响程度。结果方差分析显示A、B组CA199显著高于C、D组,且A组更高于B组;Spearman相关分析显 示CA199水平与FBG(r = 0. 179,/> =0. 016)、HbA,C( r =0. 424,/? <0. 001)呈正相关;多元线性回归分析显示: HbAf是影响lnCA199的独立危险因素08=0.388,P<0.001)。结论血清CA199在2型糖尿病患者中有低水 平升高,考虑可能与血糖长期控制不佳密切相关。 |

Objective To investigate the variance of serum CA199 levels and its related factors in type 2 diabetic patients. Methods One hundred and eighty hospitalized type 2 diabetic patients were enrolled in the study and were divided into 3 groups according to HbAlc levels : group A (HbA^ > 7.5% , n =56),group B (HbAlC 6.5%-7.5%,n = 59)and group C (HbAlC <6. 5%,n =65); and 60 healthy subjects were also enrolled as normal controls (group D). The levels of serum CA199, fasting blood glucose, glycosylated hemoglobin, fasting insulin, fasting C-peptide, age,sex, body mass index, waist circumference, total cholesterol, triglyceride, high density lipoprotein cholesterol,low density lipoprotein cholesterol and other variables were used for analysis. The levels of serum CA199 were compared among the 4 groups, and the correlation of CA199 with other variables was analyzed with multiple linear regression. Results CA199 levels in groups A and B were significantly higher than groups C and D,and that of group A was the highest. Spearman relation analysis showed that the CA199 level was positively correlated with FBG (r =0.179,P =0.016),HbA^ (r = 0.424, P <0.001). Multiple linear regression analysis showed that HbAjC was the independent risk factor for increased CA19 ()8 =0.388,P <0.001). Conclusion The low-level elevation of serum CA199 in type 2 diabetic patients may be closely related to long-term poor glucose control. |
