目的分析消化道恶性肿瘤患者术前营养状态与生活质量的相关性。方法对拟初次接受手术治疗的108例消化道恶性肿瘤患者术前应用主观全面评价法进行营养状况评估,并应用癌症患者生活质量核心问卷表进行生活质量评价。结果营养状态良好的患者为61例,占56.48% ;轻-中度营养不良的患者为40例,占37.04%;重度营养不良的患者为7例,占6.48%。营养状态与癌症分期具有相关性r=0.240,P<0.05)。不同营养状态患者比较,整体生存质量、躯体、角色、认知功能、疲乏、恶心呕吐、食欲丧失症状、社会功能、呼吸困难、睡眠障碍差异有统计学意义(P<0.05);而情绪功能、疼痛、便秘、腹泻、经济困难等差异无统计学意义(P>0.05)。整体生存质 量、躯体、負色、认知功能、疲乏、恶心呕吐、食欲丧失症状、情绪功能、呼吸困难、睡眠障碍、便秘、经济困难与术前营 养状态有相关性(P<0.05 );而社会功能、疼痛、腹泻症状与术前营养状态无相关性(P>0.05 )。结论消化系统恶性肿瘤患者营养不良发生率高而且生活质量差,癌症分期越高营养状态越差,营养状态影响消化道恶性肿瘤患者的生活质量。 |

Objective To analyze the association between nutritional status and quality of life before operation in patients with digestive cancers. Methods The nutritional status was evaluated with objective global assessment and the quality of life was assessed with Cancer Core Questionnaire of Quality of Life in 108 patients with digestive malignant tumors before the first operation. Results There were 61 cases in good nutritional status (56.48% ), 40 cases in mild-moderate malnutrition (37.04% ) and 7 cases in severe malnutrition (6.48% ). There was a correlation between the nutritional status and the stage of the disease (r=0.240, P<0.05). There were significant differences in global functioning, physical, role, cognitive function, fatigue, nausea/vomiting, appetite loss, social function, dyspnea and sleepless (P<0.05 ) among patients with different nutritional status; while there were no significant differences in emotional functioning, pain, constipation, diarrhea and financial difficulties (P>0.05). The nutritional status before operation was significantly correlated with global functioning, physical,role,cognitive function,fatigue,nausea/ vomiting, appetite loss,emotional function,dyspnea, sleepless, constipation and financial difficulties and (P<0.05 ) ; and not correlated with social functioning, pain, and diarrhea (P>0.05 ). Conclusion The nutritional status are associated with the quality of life in patients with digestive cancers,and the nutritional status is also correlated with the clinical stages of the cancer patients. |
