目的探讨双源CT冠状动脉造影评价冠心病无症状人群的预后价值。方法连续录入392例健康体检患者,行冠状动脉CT造影(coronary computed tomographic angiography, CCTA)后,分析CCTA结果对患者预后的预测价值。将CCTA结果阳性患者随机分为A、B组。A组为对照组,无特殊处理。B组为处理组,予强化药物治疗。随访6~12个月。结果392例受试者中共有278例(71%)发现斑块,82例为CCTA阳性。有梗阻性狭窄患者FRS评分显著高于非梗阻性狭窄患者FRS评分(11.76±4.55 vs 8.68±4.82,P=0.00)。ROC曲线FRS评分预测患者阳性斑块,线下面积为0.683。A、B两组之间心血管事件的发生率无显著差异。结论CCTA对无冠心病无症状高危人群的短期预后有意义。 |

Abstract: |
Objective To assess the prognostic value of coronary CT angiography (CCTA) in asymptomatic patients with coronary artery disease. Methods Total 392 asymptomatic patients underwent CCTA examination from December 2010 to June 2011. Patients with positive CCTA results were randomly divided into 2 groups: patients in group A did not have special treatment and those in group B received drug treatment. All patients were plaques were found in 278 cases (71%), among followed up for 6 - 12 months. Results Coronary whom 82 were CCTA positive. Framingham risk score (FRS) scores in coronary obstructive stenosis patients were significantly higher than those with non-obstructive stenosis ( 11.76 ± 4. 55 vs 8.68 ±4. 82, P = 0. 00). The ROC area under the curve of FRS in prediction of positive patients with plaque was 0. 683. There was no significant difference in incidence of cardiac events between groups A and B. Conclusion CCTA is of value in short-term prognosis for asymptomatic and high risk patients with coronary heart disease. |
