Z-G旁流式EtCO2监测吸氧装置的准确性及其临床应用

摘要:
目的评价Z-G旁流式EtCO2监测吸氧装置EtCO2监测准确性及其在非气管插管全身麻醉中的应用效果。方法乳房肿块切除女性患者100例,予非气管插管全身麻醉,术中使用旁流式非气管插管EtCO2监测吸氧装置。予咪达唑仑、芬太尼、异丙酚行静脉麻醉保留自主呼吸,记录给药前(T0)、给药后EtCO2波形稳定后5min(T1)、EtCO2波形稳定后10min(T2)的MAP、HR、SpO2、RR、PETCO2;分别于各时间点抽取动脉血行血气分析。结果麻醉前PETC02为(33.25±1.34)mmHg,P。C02为(36.20±1.15)mmHg。术中PETCO2、PaCO2、pH在Tl、T2显示轻度的呼吸性酸中毒的表现。PETCO2与PACO2相关系数r=0.627,呈正的直线相关,Pa-etCO2平均为3.77mmHg。所有患者的点在95%一致性界限以内,一致性界限以内Pa-etCO2最大值为6.2mmHg,PETCO2和PACO2的平均值为38.5mmHg。结论Z-G旁流式EtCO2监测吸氧装置监测EtCO2使用方便,PETCo2与PaCO2相关性好,可以及早、迅速、准确的发现呼吸异常,值得推荐使用。

Abstract:
Objective To evaluate the accuracy and clinical effect of Z-G sidestream non- endotracheal intubation oxygen device in monitoring EtCO2. Methods One hundred female patients underwent breast lump resection. The non-endotracheal intubation anesthesia and the Z-G sidestream non-endotracheal intubation oxygen device were applied in operation. Midazolam, fentanyl and propofol was given for intravenous anesthesia with spontaneous respiration and MAP, HR, SpO2, RR, PETCO2 were recorded immediately before administration (T0) ; 5 min (T1 ) and 10 min (T2 ) after the waveform of EtCO: was stable. At each time point, arterial blood was collected for blood gas analysis. Results PETCO2 was (33.25 ±1.34) mmHg, paCO2 was (36.20 ±1.15 ) mmHg before anesthesia. Intra-operative PETCO2. paCO2, pH at T1and T2 showed slight respiratory acidosis. The correlation coefficient of PETCO2 and PaCO2 was 0. 627 with a positive linear correlation ; the average value of Pa- etCO2 was 3.77 mmHg. All points were in 95% consistency within boundaries, the maximal Pa-etCO2 was 6. 2 mmHg within boundaries, the average value of PET CO2 and pa CO2 was 38. 5 mmHg. Conclusion Both the effect of monitoring and offering oxygen is exact when using the Z-G sidestream non-intubation device. The abnormal breathing can be found early, rapidly and accurately when using this device. PETCO2 is positively correlated with paCO2. EtCO2 and p, CO2 has good consistency and high accuracy, so it is worth promoting.
