帕金森病(Parkinson’s disease, PD)是中老年人常见的神经系统退行性疾病,其主要临床症状包括运动迟缓、静止性震颤、肌强直等运动症状和嗅觉减退、便秘、抑郁等非运动症状[1]。研究表明,PD患者存在体适能下降,约为健康同龄人的1/3,具体表现为心肺耐力、肌肉力量、柔韧性及平衡功能下降[2-6];此外,PD患者易出现姿势步态障碍,如持续性或发作性的慌张步态和冻结步态(freezing of gait, FOG)等[7]。因此,PD患者跌倒风险是健康老年人群的3倍[6],严重影响患者的生活质量。科学运动可有效改善各阶段PD患者的运动症状和非运动症状[8-10]。
处于“开期”的PD患者,在医疗监护情况下,可完成适宜的最大强度运动或次最大强度运动。采用跑步机或功率车进行分级运动试验(graded exercise test, GXT),评估患者对递增强度运动的承受能力,同时对与运动相关不良事件风险(如血压过高、心律失常、严重心肌缺血等)进行评估与管理[11-12];对不能完成GXT者,可进行6分钟步行测试(6 minute walking test, 6MWT)。耐力评估时,应采用Borg 主观体力感等级量表(Borg’s 6-20 Rating of Perceived Exertion Scale),评估其主观用力程度(rating of perceived exertion, RPE)[13]。
根据患者运动期间的最大心率(HRmax)百分比评估其有氧运动强度(低、中和高强度),HRmax达到(220-年龄)的40%~60%,为中等强度有氧运动;HRmax达到(220-年龄)的60%~80%时,则为高强度运动。50%以上的PD患者存在心脏交感神经功能障碍,进行次最大强度运动时,心率增加不明显,这种情况下推荐Borg 主观体力感等级量表,根据RPE分数判定其运动强度(13~16分为中强度运动,如使用β受体阻滞剂,则为14~16分;17~20分为高强度运动[10])。
采用可重复一次的最大力量(one-repetition maximum, 1-RM)测量肌力[14]。考虑测试安全性,上肢肌力评估可采用静态握力测试[15]和肱二头肌屈曲测试[16];下肢肌力评估可采用5次坐立试验(the five times sit to stand test, FTSTS)[10]。抗阻运动强度由1-RM的百分比判断,1-RM的60%~80%为中等抗阻运动强度,1-RM的80%~95%为高强度抗阻运动。
应评估不同部位的柔韧性。可采用双手背勾测试评估上肢柔韧性;坐位体前屈测试评估下肢柔韧性;坐位旋转测试评估躯干柔韧性 [10]。
采用以下工具评估平衡功能:改良的帕金森活动量表(Modified Parkinson Activity Scale, M-PAS)、“起立-行走”计时测试(timed up & go test, TUGT)、简易平衡评定系统测试(mini-balance evaluation systems test, mini-BES Test)、伯格平衡量表(Berg Balance Scale,BBS)和单腿直立测试(single-leg standing test, SLST)等。建议采用2种以上工具综合评估[10]。
采用10米步行测试(10-meter walk test, 10MWT)评估步行速度;使用新版冻结步态问卷(new freezing of gait questionnaire, NFOG-Q)评估FOG的频率和严重程度[10]。
(1)建议每6~12个月进行病情进展及运动功能评估[17]。
(2)评估应在服药45~60 min后、药物效果良好时进行;有症状波动者应分别在“开期”和“关期”状态下评估。
(3)评估前热身5~10 min。
(4)评估时应采取有效的保护措施,保障患者安全。
应制定个体化的FITT-VP运动处方,包括运动频率(frequency, F)、运动强度(intensity, I)、运动类型(type, T)、运动时间(time, T)、运动总量(volume, V)及运动进度(progression, P),确保在安全的前提下获得良好的运动效果。每次练习时间应限制在30~60 min。无法耐受者应循序渐进,逐步增加运动时间和强度。
2.2.1 运动类型 选择增强心肺耐力的有氧运动、增加肌肉力量的抗阻运动以及提高关节活动能力的牵伸运动等。
(1)有氧运动。包括以下三类:① 与娱乐、体育和休闲有关的有氧运动,如快走、慢跑、骑自行车、游泳、舞蹈、跳绳、骑马、瑜伽及乒乓球、网球、保龄球、高尔夫球、足球等球类运动;② 与工作、家庭有关的有氧运动,如农事、轻负重、割草、清扫和园艺、外部涂漆、清洗窗户、清理排水沟及护理老年人(穿衣、移动)等;③ 中国传统心身锻炼气功功法,如太极、五禽戏、八段锦等。
(2)抗阻运动。包括使用运动器械(如杠铃、哑铃、壶铃、拉力器、阻力带等)进行抗阻运动,以及无须运动器械的仰卧起坐、俯卧撑和引体向上等。
(3)静态和动态拉伸运动。在静态拉伸中,采用适宜的拉伸动作拉伸某一特定肌肉,如压肩和双臂外展拉伸(肩部肌肉)、坐位体前屈(腰部肌肉)、坐压腿和直膝分腿(腿部肌肉)等;在动态拉伸中,可选择合适的动作,重复主动拉伸特定肌肉。
2.2.2 运动强度 目前,PD各阶段的最佳运动强度仍不明确,但较高强度的运动改善运动症状更显著[18]。在身体适应和安全的情况下,抗阻运动应从中强度开始,逐步增加阻力、提高速度、增加重复次数;有氧运动时,应逐步增加运动时间或提高运动时HRmax的百分比,以中-高强度运动为目标。应逐步增加运动强度,避免运动处方功效不足;牵伸时应尽可能伸展、弯曲或旋转至身体可承受的最大程度,并在牵伸动作的末端做短时间的保持,以达到牵伸效果。
2.2.3 运动时间和频率 每日进行至少30 min的运动。如果难以坚持30 min,可分3次,每次10 min。每周应完成150~300 min的中强度有氧运动,或至少75~150 min的高强度有氧运动,或等量的中及高强度的运动组合。每周至少2 d进行涉及主要肌群的抗阻运动,中等强度抗阻运动3~5组,每组重复10次,或高强度抗阻运动3组,每组重复8次。每日进行牵伸运动20~30 min[19-20]。

3.1.1 重视运动前评估 对严重精神障碍、复杂的运动并发症、DBS植入、运动期间出现胸痛、心律失常或血压不规则变化、控制不佳的2型糖尿病、急性全身性疾病、慢性阻塞性肺疾病、严重的心血管损害等,应进行严格评估。在保证安全的前提下,制定运动方案,防止摔伤、骨折等意外事件发生。运动前后应做好热身和整理运动。
3.1.2 采用综合性运动方案 推荐多种类、分段式的运动方案。如以1周为循环,则第1天做增强肌肉力量的运动,第2天做提高心肺耐力的运动,第3天做增加关节柔韧性的运动。
3.1.3 兼顾“开期”和“关期” 由“开期”开始运动,逐步扩展到“关期”,以达到更好的运动效果;也推荐根据患者开关期的运动功能不同而设定不同的运动处方,相应进行适宜训练。
3.1.4 鼓励“康护”多学科合作 晚期(H-Y 5期)的治疗重点在于维持基本的生活能力,防止并发症,如压疮、挛缩和畸形等。护理人员应协助其运动,从被动运动开始,逐步过渡到助力运动、主动运动,以维持关节活动度和肌肉容积、减少骨折、肌肉损伤等并发症为主要目标。运动时应避免体能减退。
运动时出现恶心、胸闷或胸痛、呼吸困难、严重疲劳、头晕或眩晕、心悸、疼痛、冷汗、监测发现运动期间收缩压降低超过10 mmHg(1 mmHg=0.133 kPa)等,均须立刻终止运动,寻求医疗救助。
综上所述,运动是PD全程管理中的重要措施。PD运动处方专家共识写作组依据FITT-VP运动处方制定原则,秉承“持之以恒、动之有方”的运动理念,充分考虑临床适用性制定该共识。建议在该共识基础上,制定适宜的运动处方,并及时评估、优化,实现最佳运动效果。
