超声造影技术评价慢性肾脏病患者肾脏纤维化的价值

摘要:
目的探讨超声造影技术在慢性肾脏病肾脏纤维化中的评估价值。方法采用超声造影技术检测25例患者穿刺侧肾脏,肾组织Masson染色评估纤维化程度,进行相关性分析。结果25例肾穿刺患者以肾脏病理进行纤维化分级,估算肾小球滤过率与肾脏纤维化面积呈负相关(r=-0.65,P<0.05);定量分析超声参数上升时间和平均渡越时间与肾脏纤维化面积有相关性,相关系数分别为0.45和0.49(均P<0.05)。最大峰值强度和上升时间在T3组和T0组差异有统计学意义(P均<0.05)。结论超声造影参数上升时间和平均渡越时间与肾组织纤维化面积有相关性,超声造影技术在肾脏纤维化无创评估中具有诊断价值。

Abstract:
ObjectiveTo evaluate the application of contrast-enhanced ultrasonography in diagnosis of renal fibrosis in patients with chronic kidney disease. MethodsTwenty-five patients with chronic renal disease who underwent renal biopsy were enrolled in the study. The contrast-enhanced ultrasonogrphy(CEUS) was used to evaluate thefibrosis of kidney, and the correlation between CEUS parameters and the degree of renal fibrosis was analyzed. ResultsAll the 25 renal biopsy tissues were graded by renal pathology And the eGFR was negatively correlated with renal fibrosis area(r=-0.65, P<0.05). The rising time and mean transit time of CEUS were positively correlated with the fibrosis area(r=0.45,P=0.023 and r=0.49, P=0.013, respectively). There were significant differences in maximum peak intensity and rising time between T3 and T0 groups (P<0.05 respectively). ConclusionThe rising time and mean transit time are correlated with renal fibrosis area in CEUS, indicating that CEUS can be used as a non-invasive diagnostic method for evaluation of renal fibrosis in patients with chronic renal disease.
