摘要:

亚临床舒张功能障碍（preclinical diastolic dysfunction, PDD）以心肌主动松弛能力受损和顺应性降低为主要特点，导致左心室舒张末期压力增高，进而造成心脏功能受损及病理性结构异常。当前我国中老年PDD患病率为30%~40%，在人群中较为常见。随着左心室舒张期充盈压力的不断增加，PDD患者的心脏功能及结构异常会进一步加重，最终可能导致心力衰竭的发生。因此，本文从PDD的临床特点、流行病学现况及其与心力衰竭发病关联的角度进行探讨，以期为心力衰竭的早期预防提供新的视角，建构更具实用性的心力衰竭危险人群的早期干预策略。

Abstract:

Preclinical diastolic dysfunction（PDD) is characterized by impaired active relaxation and reduced compliance of myocardium, leading to increased left ventricular（LV) end-diastolic pressure and causing cardiac dysfunction and structural abnormality. The current prevalence of PDD in elderly population in China is about 30%-40%. As the LV diastolic filling pressure continues to increase, the cardiac dysfunction and structural abnormalities of PDD patients are further aggravated, eventually leading to heart failure. This article reviews the clinical characteristics, epidemiology of PDD and its association with heart failure, in order to provide a perspective for early prevention and intervention of heart failure in risk population.
