轻度认知障碍患者嗅觉功能损害的磁共振弥散张量成像研究

摘要:
目的探讨轻度认知障碍(mild cognitive impairment, MCI)患者嗅觉功能与脑白质改变之间的关系。方法对受试者采用蒙特利尔认知量表(Montreal cognitive assessment, MoCA)进行认知评估,70%异丙醇试纸进行嗅觉功能测试,应用基于纤维束示踪的空间统计方法(tract-based spatial statistics, TBSS)得到弥散张量成像(diffusion tensor imaging, DTI)相关参数,比较36例MCI患者及32例正常老年人行为学及影像学结果。结果MCI患者的嗅觉评分普遍降低,与MoCA评分呈正相关(r=0.682,P<0.05)。DTI参数中各向异性分数(fractional anisotropy, FA)变化较为显著,FA减弱区主要位于胼胝体周围、额叶眶额回周围、左侧枕叶周围区域,其中嗅觉评分与额叶眶额回的FA值相关性较为显著(P<0.05)。结论本组MCI患者嗅觉功能下降,且与神经心理学量表评分呈正相关。MCI患者嗅觉相关脑区脑白质损害,记忆相关脑区脑白质损害不明显,提示嗅觉功能下降可能早于记忆功能受损。

Abstract:
ObjectiveTo explore the relationship between olfactory function and white matter changes in patients with mild cognitive impairment(MCI). MethodsThe Montreal cognitive assessment(MoCA) was used for cognitive assessment, and 70% isopropanol test paper was used for olfactory function test. Based on the diffusion tensor imaging(DTI) technique, the tract-based spatial statistics(TBSS) was used to obtain the relevant parameters. Then compared the behavior and imaging results of 36 MCI patients and 32 normal elderly patients. ResultsThe olfactory function of MCI patients generally decreased, which was positively correlated with the MoCA score(r=0.682, P<0.05). Fractional Anisotropy(FA) changes significantly. The weak area of FA was mainly located around the corpus callosum, the frontal orbital gyrus and the left occipital lobe. The olfactory score was significantly correlated with the FA value of the inferior frontal gyrus(P<0.05). ConclusionThe olfactory function in MCI patients is decreased, which is positively correlated with neuropsychological scale scores. The damage of white matter in the area of olfactory function is obvious. However, the damage of white matter in the area of memory and cognition function is not obvious. It suggests that the decline in olfactory function may be earlier than cognitive function impairment.
