时间空间标记反转脉冲技术在门静脉无创成像的价值分析

摘要:
目的评估时间-空间标记反转脉冲(time-spatial labeling inversion pulse, Time-SLIP)血管成像的成像质量,评价Time-SLIP和对比增强磁共振成像(contrast-enhanced magnetic resonance portography, CE-MRP)在肝硬化门脉高压症评估方面的差异。方法回顾性收集上海市浦东新区公利医院2018年1月—2020年1月期间收治的60例肝硬化患者的临床及磁共振成像(magnetic resonance imaging, MRI)资料。根据成像方式分为门静脉Time-SLIP组及CE-MRP组,对门静脉图像质量及解剖分级进行评分,所有病例均采用双盲法对图像进行评分。采用Wilcoxon配对秩和检验分析Time-SLIP与CE-MRP的门静脉图像质量及解剖分级显示差异是否存在统计学意义。结果60例肝硬化患者的Time-SLIP与CE-MRP门静脉图像质量差异无统计学意义(P>0.05),两组的门静脉解剖分级部分差异存在统计学意义(P<0.05),其中5个研究对象Time-SLIP解剖分级为6级,而CE-MRP解剖分级为5级。结论Time-SLIP检查技术在肝硬化患者门静脉血管成像中可以获得较好的图像质量及门静脉解剖分级显示,可以作为CE-MRP的替代检查方法,具有较好的临床应用及诊断价值。

Abstract:
ObjectiveTo evaluate the imaging quality of time-spatial labeling inversion pulse (Time-SLIP) angiography in patients with cirrhotic portal hypertension. MethodsThe clinical and imaging data of 60 patients with liver cirrhosis admitted to Gongli Hospital of Pudong New Area from January 2018 to January 2020 were retrospectively analyzed. All patients underwent Time-SLIP and contrast-enhanced magnetic resonance portography (CE-MRP) examinations. The image quality and anatomical grading of portal vein by Time-SLIP and CE-MRP were independently scored with double-blind method, and compared between two methods with Wilcoxon matched-pairs ranks test. ResultsThere were no statistical differences in the image quality between Time-SLIP and CE-MRP in 60 patients with liver cirrhosis (P>0.05). However, there were significant differences in the anatomy grading (P<0.05); 5 patients were rated as grade 6 in Time-SLIP, while as grade 5 in CE-MRP. ConclusionTime-SLIP technique can obtain good image quality and portal vein anatomic grading of patients with liver cirrhosis. It can be used as an alternative method of CE-MRP with good clinical application and diagnostic value.
