上海市社区精神分裂症患者社会经济状况及影响因素分析

摘要:
目的调查上海市社区精神分裂症患者的工作、经济和生活状态,并探讨相关人口学影响因素。方法使用自编患者社会经济基本状况调查表,调查基于重性精神障碍管理系统、随机抽样选取的社区精神分裂症患者,了解其社会经济状况。结果入组社区精神分裂症患者样本333例(男性183例,女性150例)。患者中72.8%为无业,74.3%月收入在2000元以下,仅有22.8%以固定工资为主要收入;社区分裂症患者中84.0%以社会医保为主要医疗支付方式,68.3%的患者基本能达到收支平衡。回归分析发现,就业、经济情况与患者的健康与生活水平正相关;已婚、已育患者的就业、经济水平更高;使用一代抗精神病药的患者就业率更低。女性、已婚、已育、受教育程度高者对未来的经济状况更加乐观(均P<0.05)。结论社区精神分裂症患者的工作状况和社会经济水平较低,尤其男性、未婚、低教育水平患者状况更为严峻,需加强有针对性、个体化的康复干预。

Socioeconomic status of community dweller with schizophrenia in Shanghai and its influencing factors

Abstract:
ObjectiveTo investigate the socioeconomic status of patients with schizophrenia and their influencing factors. MethodsThe outpatients with schizophrenia were randomly selected from communities in Shanghai for the study. The socioeconomic status of patients was surveyed with a self-designed questionnaire. ResultsThe survey showed that among 333 participants (183 male and 150 female), 72.8% were unemployed, only 22.8% patients had regular salary as their main income, and the income of 74.3% patients was below 2000 Yuan per month, 68.3% patients were able to make both ends meet, and 84.0% patients had social insurance to cover their medical expenditure. Multivariate regulation analysis showed that employment and financial condition were associated with the self-evaluation of health and living condition., while being married and having children were associated with higher employment and financial status; patients taking first-generation antipsychotics suffered a higher unemployed rate; female, married patient with children and patients with higher education were more optimistic about their future finical status (all P<0.05). ConclusionPatients with schizophrenia suffer from unemployment, low income, and sexual dysfunction, thus targeted individualized interventions are needed.
