阿尔茨海默病患者选择性损害海马后部的MRI研究

摘要:
目的探讨阿尔茨海默病(Alzheimer's diseases, AD)患者左右侧海马前后部体积改变。方法基于MRI的3D高分辨率结构数据,利用体素形态测量学(voxel-based morphometry, VBM)方法对全脑灰质进行分割,提取海马体积,利用Brainnetome atlas模板将左右侧海马分割成前部和后部两部分。对比分析27例AD患者及22例正常对照左右侧海马前后部体积变化。采用统一简明智能状态检查量表(mini-mental state examination, MMSE)进行认知能力评估。采用两独立样本t检验,年龄作为协变量,比较AD组与正常对照组左右侧海马前后部体积差异,将AD患者左右侧海马前后部体积与认知评分进行皮尔逊相关分析。结果AD患者左右侧海马后部较正常人明显萎缩(FWE矫正,P<0.001,t=3.277),而左右侧海马前部无明显差异。左右侧海马前后部体积与认知评分均相关(P<0.05),左侧海马后部体积与MMSE分相关性最大(P<0.001,r=0.412)。结论AD患者与回忆功能有关海马后部区域选择性受损,这可能为了解AD的发生机制提供新思路。

Selective damage of posterior hippocampus in Alzheimer's disease: a MRI study

Abstract:
ObjectiveTo investigate the volume changes of hippocampal subregions in patients with Alzheimer's diseases(AD) by MRI scan. MethodsTwenty-seven outpatients with AD and 22 healthy subjects were enrolled in the study. Based on MRI-3D high-resolution structural data, the voxel-based morphometry(VBM) method was applied to segment the whole brain gray matter and extract the hippocampal volume. The left and right hippocampus were split into the anterior and posterior parts using the Brainnetome atlas template. The cognitive ability was evaluated with mini-mental state examination(MMSE) scale. Two-sample t tests were used to compare the bilateral anterior and posterior hippocampal volume differences between the AD patients and the normal controls. The correlation of cognitive scores(MMSE) with hippocampal volume was analyzed. ResultsThe bilateral posterior hippocampus of AD patients was significantly atrophied compared with normal controls(FWE correction, t=3.277, P<0.001), while there was no significant difference in the volume of the bilateral anterior hippocampus between two groups. The volume of each hippocampal subregion was correlated with the MMSE score(P<0.05), particularly for left hippocampus(P<0.001,r=0.412). ConclusionThe posterior hippocampus is selectively damaged in patients with AD and these findings may provide new clues for understanding the possible mechanism of AD.
