上海某公立医院新型冠状病毒肺炎防控期间发热门诊患者体验度测评研究

摘要:
目的探讨新型冠状病毒肺炎(coronavirus disease 2019, COVID-19)疫情防控期间影响发热门诊患者体验相关因素,为进一步提高发热门诊医疗服务质量提供参考。方法构建《上海市第六人民医院东院发热门诊患者体验度测评表》,对2020年2月至3月的发热门诊患者开展问卷测评,进行描述性统计分析和相关性分析,并检验其信度和效度。结果测评表包括27个条目,专家效度S-CVI为1.00,各条目I-CVI均为1.00;通过最大正交旋转法共得到3个维度:防控审核措施、服务质量和医疗成效,累计贡献率为64.707%,总量表的Cronbach's α系数为0.902,各维度的Cronbach's α系数为0.844~0.901。测评结果显示体验度最低的4项依次为:“疫情期间,对我院就诊流程的了解”得分为4.146;“对我院推出的就诊流程视频的评价”的得分为4.472;“对居住史、旅行史和接触史调查的评价”的得分为4.623;“对所有来院人员都要测量体温流程的评价”的得分为4.722。此外,“疫情期间,对我院就诊流程的了解”“对实名制就诊的评价”“对流行病学调查流程的评价”在内的6项指标的评分显著相关。结论应结合重点人群就医体验,优化防控措施审核前的宣传渠道覆盖,增加现场工作人员沟通能力,提升COVID-19疫情期间发热门诊医疗质量和患者体验度。

Abstract:
ObjectiveTo develop an evaluation scale of outpatient experience in the fever clinic of Shanghai Sixth People's Hospital East Campus during the epidemic of coronavirus disease 2019(COVID-19). MethodsThe evaluation scale of outpatient experience in the fever clinic of Shanghai Sixth People's Hospital East Campus was developed, and tested among outpatients visiting the fever clinic from February to March 2020. The descriptive statistical analysis and correlation analysis were carried out, and the reliability and validity of the scale were also assessed in this study. ResultsThe evaluation scale included 27 items. The S-CVI was 1.00, and each I-CVI was 1.00. Three factors prevention and control audit measures, service quality and medical efficiency, were extracted from items by the factor analysis via the maximum orthogonal rotation; the cumulative contribution rate was 64.707%. Cronbach's α coefficient for the scale was 0.902, Cronbach's α coefficient for three dimensions was 0.844-0.901. According to the evaluation results, the four items with the lowest degree of experience were in the following order:“understanding of the treatment process in our hospital during the epidemic” was scored as 4.146; “evaluation on the video of medical treatment process launched by our hospital” was scored as 4.472; “evaluation of survey on residence history, travel history and contact history” was scored as 4.623; “assessment of the temperature process for all admissions” was scored as 4.722. In addition, a significant correlation was presented among the scores of “understanding of the treatment process in our hospital during the epidemic”, “evaluation of real-name treatment system”, “evaluation of epidemiological investigation process” and other 6 indicators. ConclusionConsidering the feedback from key outpatients, the publicity channel coverage should be optimized before the review of prevention and control measures, the communication ability of field staff should be enhanced, and the patient experience and service level of fever clinics should also be improved.
