胸腰椎有限元模型的建立及椎管内骨折块间接复位的生物力学分析

摘要:
目的 建立T12~L2节段有限元模型,模拟椎管内骨折块在不同占位比例下,比较牵伸与非牵伸椎体组后纵韧带回缩力,探索骨折块的间接复位机制。方法 通过螺旋CT扫描获得胸腰椎区域图像,导入Mimics软件获得三维几何模型;应用有限元软件Abaqus,建立正常T12~L2节段有限元模型和L1骨折块有限元模型;将模型分为牵伸T12椎体2mm组和非牵伸组(以下简称牵伸组和非牵伸组),分别设置50%、40%、30%、20%、10%的骨折块椎管内矢状直径占位比(sagittal canal diameter occupying ratio, SCDOR),比较两组后纵韧带的弹性回缩力。结果 建立了有效的T12~L2节段有限元模型;当SCDOR为50%、40%时,牵伸组与非牵伸组韧带回缩力分别为92.14N、95.25N和82.33N、84.99N,两组差异较小;当SCDOR小于30%时,牵伸组回缩力明显高于非牵伸组。结论 建立了有效的T12~L2节段有限元模型,可用于有限元分析。当SCDOR在40%以上时,牵伸椎体对增加后纵韧带回缩力的作用有限。SCDOR在30%以下时,牵伸椎体可明显增加后纵韧带回缩力,有利于骨折块的复位。

Abstract:
Objective To establish the finite element model of thoracolumbar spine and to analyze the indirect reduction of intraspinal fracture block. Methods The images of thoracolumbar spine were obtained by CT scan, and the mimics software was used to obtain the 3D geometric model. The software Abaqus was applied to establish normal T12-L2 segment finite element model and L1 fracture block finite element model. The model was divided into 2mm upward stretched group and the unstretched group. The 50%, 40%, 30%, 20%, 10% of the sagittal canal diameter occupying ratio(SCDOR) were set, and the retractive forces of the posterior longitudinal ligaments were compared. Results The effective T12-L2 segment finite element model was established. When the SCDOR was 50% and 40%, the ligament retraction force of the unstretched and stretched group was 92.14N, 95.25N and 82.33N, 84.99N, respectively(both P>0.05). When the SCDOR was <30%, the retraction force of the stretched group was significantly higher than that of the unstretched group. Conclusion An effective T12-L2 segment finite element model has been established in the study. When SCDOR was above 40%, the effect of stretching the vertebral body on increasing the posterior longitudinal ligament retraction force is limited. When the SCDOR was below 30%, the vertebral body can significantly increase the retraction force of the posterior longitudinal ligament, which is beneficial to the reduction of the fracture block.
