3D打印导板在关节镜下膝前交叉韧带重建中的临床应用

摘要:
目的 比较在3D打印技术辅助下行前交叉韧带(anterior cruciate ligament, ACL)重建与传统治疗方案在手术时间、股骨骨隧道长度、术后近期疗效的差异。方法 选取2017年6月—2018年5月收治的ACL断裂患者30例,随机分为两组,每组15例,观察组予以关节镜下3D导板辅助ACL重建手术治疗,另外一组为对照组(传统重建组),两组均采用单束重建,观察对比两组患者手术时间、股骨骨隧道长度及术后Lysholm膝关节评分(术后疗效评价)。结果 全部患者均随访3个月以上,观察组手术时间(46.3±12.2)min,股骨骨隧道长度(42.47±3.248)mm,术后3个月Lysholm膝关节评分(95.35±5.05)分;对照组手术时间(61.5±20.2)min,股骨骨隧道长度(42.67±5.678)mm,观察组骨隧道较对照组集中。Lysholm膝关节评分(88.18±9.80)分;其中观察组的手术时间和术后Lysholm膝关节评分较对照组差异有统计学意义(P<0.05),而平均住院日差异无统计学意义(P>0.05)。结论 3D导板辅助下前交叉韧带重建手术时间更短,术后膝关节Lysholm评分更优,股骨骨隧道长度更稳定可控。

Abstract:
Objective To assess the application of 3D-printed guide plates in arthroscopic anterior cruciate ligament(ACL) reconstruction. Methods Thirty patients with ACL fractures admitted from June to May 2017 were randomly divided into two groups with 15 in each group. The study group received the arthroscopic 3D-guide plate assisted ACL reconstruction and the control group received the conventional method; the single-bundle reconstruction was performed in both groups. The operation time, length of femoral bone tunnel and postoperative Lysholm knee joint score were compared between the two groups. Results All patients were followed up for more than 3 months. The operation time of the study group was shorter [(46.3±12.2)min vs (61.5±20.2)min, P<0.05] and the Lysholm knee score at the 3 months postoperatively was higher [(95.35±5.05) vs (88.18±9.80), P<0.05] than those of control group. The length of the femoral tunnel of two groups was (42.47±3.248)mm and (42.67±5.678)mm, respectively. There was no significant difference in length of hospital stay between two groups. Conclusion The efficacy of 3D-guide plate assisted the reconstruction of the anterior cruciate ligament is better than the conventional method in treatment of patients with ACL fractures.
