钬激光在男性难治性肛管尖锐湿疣治疗中的应用

摘要:
目的 探讨钬激光治疗肛管尖锐湿疣的临床疗效及安全性。方法 回顾性收集2017年5月—2019年9月在上海市皮肤病医院光医学治疗科接受钬激光27例男性肛管尖锐湿疣患者作为治疗组,另外,选择同期接受CO2激光的35例男性患者作为对照组进行研究,观察肛管内疣体清除率、复发率及不良反应。结果 单次钬激光疣体清除有效率(85.2%)高于CO2激光(54.3%),差异有统计学意义(P=0.005);6个月随访中钬激光治疗组的疣体复发率(25.9%)明显低于CO2激光组(51.4%,P=0.042)。两组患者治疗后均有局部水肿、浅溃疡等反应,2~3周痊愈,未见感染、瘢痕、排便困难等现象。结论 钬激光治疗肛管疣体清除率高、复发率低,副作用小,值得临床推广应用。

Application of holmium laser in treatment of refractory anal canal condylomata acuminata in male patients

Abstract:
Objective To investigate the efficacy and safety of holmium laser in treatment of anal canal condylomata acuminata in male patients. Methods Twenty-seven male patients with anal canal condylomata acuminata treated with holmium laser in Department of Photomedicine at Shanghai Dermatology Hospital from May 2017 to September 2019 were enrolled as study group and thirty-five male patients treated with CO2 laser in the same period were selected as control group. The wart clearance, recurrence rate and adverse reactions were evaluated and compared in two groups. Results The effective rate of a single holmium laser treatment(85.2%) was higher than that of CO2 laser(54.3%,P=0.005). The recurrence rate of holmium laser group(25.9%) was significantly lower than that of CO2 laser group(51.4%) in the 6 months follow up(P=0.042). All two groups have local edema and anal ulcer which were healed in two to three weeks. Adverse reactions such as infection, scar formation and defecate disfunction were not found in both groups. Conclusion Holmium laser in the treatment of anal canal condylomata acuminata has a high wart clearance, low recurrence rate and less side effect, which is worth to be popularized.
