共生菌与口腔扁平苔藓的相关性研究

摘要:
目的 探索口腔共生菌在口腔扁平苔藓(oral lichen planus, OLP)的发生发展中的作用及其机制。方法 收集OLP患者及正常对照组(48例)颊拭子,采用高通量测序方法检测OLP患者颊黏膜菌群结构,并通过qPCR方法进行验证;检测OLP病损组织中产黑普氏菌及细胞炎症因子的含量,并分析其相关性。结果 OLP患者颊黏膜菌群多样性大于正常对照组,且其菌群结构与正常对照组显著不同,差异有统计学意义(P<0.05);产黑普氏菌在OLP患者颊黏膜表面菌群构成比高于正常对照组,且在门、科、属、种各层次上的差异均有统计学意义(P<0.05);产黑普氏菌在OLP患者病损组织中含量升高,且与IL-6呈正相关(P<0.05)。结论 共生菌可能在促进口腔扁平苔藓免疫失衡中发挥重要作用,其具体机制值得深入研究。

Abstract:
Objective To explore the role and mechanism of oral symbiotic bacteria in the development of oral lichen planus (OLP). Methods The buccal swabs of 43 patients with OLP and 48 normal controls were collected. The bacterial community structure of the buccal mucosa in patients with OLP was detected by high-throughput sequencing and verified by qPCR. The contents of Prevotella melaninogenica (Pm) and cytokines in lesions of OLP and normal tissues were detected by qPCR and their correlation was analyzed. Results The diversity of buccal mucosa in patients with OLP was significantly higher than that of normal controls. The bacterial structure of OLP was significantly different from that of normal controls(P<0.05). The bacteria flora showed significantly more abundant in OLP at the levels of the phylum, family, genus and species(P<0.05). The content of Prevotella melaninogenica in the OLP lesion was also increased and positively correlated with IL-6 contents(P<0.05). Conclusion Oral symbiotic bacteria may cause the immune imbalance, resulting the development of OLP. The specific mechanism need to be further studied.
