目的 探讨分析SPECT/CT骨显像应用在肿瘤患者术后随访中的价值。方法 回顾性分析104例均已行骨显像及骨密度检查的患者,其中男16例,女88例,平均年龄(62.63±13.08)岁。以骨密度诊断结果结合患者体征及临床随访结果作为诊断骨质疏松的金标准,对比骨显像及骨密度检查结果的一致性。分析骨显像及骨密度测定对于诊断骨质疏松的正确率、特异度、灵敏度。根据肿瘤患者行SPECT/CT骨显像检出的骨转移阳性率,结合临床随访及病理结果确诊骨转移的情况,综合分析SPECT/CT骨显像检查作为肿瘤患者随访项目的临床价值。结果 骨显像与骨密度对于诊断骨质疏松进行一致性检验,Kappa系数为0.738。骨显像诊断骨质疏松的准确度为92%,特异度为75%,灵敏度为96%。SPECT/CT骨显像发现肿瘤患者骨转移阳性率为36.3%。结论 SPECT/CT骨显像与骨密度测定对诊断骨质疏松有较好的一致性,诊断骨质疏松的准确度及灵敏度较高,可替代肿瘤患者随访中的骨密度测定来诊断骨质疏松。 |

Abstract: |
Objective To assess the value of SPECT/CT bone imaging in the follow-up of tumor patients.
Methods Clinical data of 104 tumor patients, including 16 males and 88 females with an average age of (62.63±13.08) years, who underwent bone imaging and bone mineral density examination, were retrospectively reviewed. The bone mineral density combined with the patient’s signs and clinical follow-up results were used as the gold standard for the diagnosis of osteoporosis, and the diagnostic consistency between bone imaging and bone mineral density was examined. The correct rate, specificity and sensitivity of bone imaging and bone mineral density for the diagnosis of osteoporosis were analyzed. According to bone metastasis detected by SPECT/CT bone imaging, clinical and pathological results, the value of SPECT/CT bone imaging in follow-up of tumor patients was determined. Results Bone imaging and bone mineral density were tested for the diagnosis of osteoporosis with a Kappa coefficient of 0.738. Bone imaging had an accuracy of 92% for diagnosis of osteoporosis with a specificity of 75% and a sensitivity of 96%. The bone metastasis rate detected by SPECT/CT bone imaging was 36.3% in this series of tumor patients. Conclusion SPECT/CT bone imaging and bone mineral density measurement have good consistency in the diagnosis of osteoporosis, and the accuracy rate and sensitivity rate are high. It may replace the bone mineral density measurement in the follow-up of tumor patients for diagnosis of osteoporosis. |
