职业倦怠对医患关系的影响

摘要:
目的 探索医生职业倦怠对医患关系的影响,为缓解职业倦怠和改善医患关系提供参考。方法 在上海10家医院选取592名医生,用Maslach职业倦怠问卷和自编医患关系问卷进行测评,并用SPSS 17.0软件对问卷调查数据进行统计分析。结果 医生对医患关系的总体评价以“比较紧张”居多,共有304人(51.4%),262人(44.3%)关于职业感受的评价是“不好不坏,一般般”,374人(63.2%)认为患方对医生“基本信任”,494人(83.4%)认为患方“基本配合”。职业倦怠评分提示情感耗竭较严重,3个维度得分从高到低为情感耗竭(3.1±1.4)分,成就感低(2.4±1.2)分,去人性化(2.1±1.3)分。3个维度得分与医患关系的4个条目得分均正相关(r=0.139~0.442,均P<0.05),多元逐步回归结果表明,职业倦怠对医患关系有消极影响(β=0.296~0.365,P<0.01)、高学历对医患关系有积极影响(β=-0.124~-0.172,P<0.01)。结论 目前医生对医患关系的评价不乐观,医生的情感耗竭较严重,职业倦怠对医患关系有消极影响,高学历的医生对医患关系的评价更加乐观,缓解职业倦怠以及医生自身的学习提升有望成为改善医患关系的途径之一。

Impacts of job burnout on the doctor-patient relationship

Abstract:
Objective To explore the impact of job burnout on doctor-patient relationship. Methods Totally 592 doctors were recruited form 10 hospitals in Shanghai and surveyed with Maslach Burnout Inventory-Human Service Survey and doctor-patient relationship questionnaire designed by the authors. The data from the surveys were analyzed with SPSS 17.0. Results The survey showed that 304 participants evaluated the current doctor-patient relationship as “tensional”(51.4%),262(44.3%) of the participants reported that they felt about their professions as “not good not bad, just so so”, 374(63.2%) thought that the patients and their family members trusted on them, 494(83.4%) thought that the patients and their family members cooperated with them. The score of job burnout indicated that emotional exhaustion among doctors was serious, with the scores of (3.1±1.4) for emotional exhaustion, (2.4±1.2) for inefficacy, and (2.1±1.3) for depersonalization. The job burnout was correlated positively with doctor-patient relationship(r=0.139-0.442,P<0.01,P<0.05). The results of multiple stepwise regression showed that high level of burnout was negative predictor of the doctor-patient relationship(β=0.296-0.365,P<0.01) and higher education was the positive predictor of the doctor-patient relationship(β=-0.124--0.172,P<0.01). Conclusion The survey indicates that at present the emotional exhaustion of doctors is serious and job burnout has a negative impact on the doctor-patient relationship. Relieving job burnout may improve the doctor-patient relationship.
