踝肱动脉脉搏波速度与早期轻度左室射血分数保留的心功能不全的相关性

摘要:
目的探讨踝肱动脉脉搏波速度(brachial-ankle pulse wave velocity, baPWV)与早期轻度左室射血分数保留的心功能不全(cardiac insufficiency with preserved ejection fraction, CI-PEF)的相关性。方法2012年8月至11月在上海市陆家嘴社区选取180例早期轻度CI-PEF人群和501例健康人群。采集所有对象的完整数据,采用简单线性相关探讨baPWV与其他指标之间的相关性;ROC曲线分析baPWV对早期轻度CI-PEF人群筛查的辅助价值。多因素Logistic回归分析早期轻度CI-PEF发生的高危因素。结果早期轻度CI-PEF患者组平均年龄大于非早期轻度CI-PEF患者组(t=-6.37,P<0.05);两组间的男女比例差异没有统计学意义(χ2=0.02,P>0.05)。两组在体质量指数BMI,baPWV差异有统计学意义。baPWV与年龄有中等强度的相关性(r=0.38);ROC曲线提示用baPWV作为早期轻度CI-PEF的早期筛查指标的研究结果(AUC=63.5,灵敏度为65.1%,特异度为57.8%)。两种多因素Logistic回归分析均显示baPWV,体质量指数,以及年龄是引起早期轻度CI-PEF的独立危险预测因子。结论baPWV对早期轻度CI-PEF具有潜在人群筛查的辅助价值。较快的baPWV、较高的体质量指数以及老龄化是引起左室射血分数保留的心功能不全高危因素。

Abstract:
ObjectiveTo investigate the association of brachial-ankle pulse wave velocity (baPWV) with early mild cardiac insufficiency with preserved ejection fraction(CI-PEF). MethodsOne hundred and eighty subjects with early CI-PEF and 501 healthy subjects were enrolled in the study from Shanghai Lujiazui community between August and November 2012. The relevant parameters were collected by echocardiography, blood sampling, physical exam and questionnaire. ROC curve was conducted to estimate the screening value of baPWV for CI-PEF and logistic regression was employed to analyze the risk factor of CI-PEF. ResultsThe mean age of subjects with CI-PEF was higher than that of healthy subjects(t=-6.37,P<0.05). There was no significant difference in gender ratio between two groups while there was significant difference in BMI, baPWV and other variables. A medium intensity correlation was indicated between the baPWV and age(r=0.38,P<0.05). ROC curve demonstrated the value of baPWV for diagnosis of cardiac insufficiency with preserved ejection fraction (AUC=63.5, sensibility=65.1%, specificity =57.8%). Multivariable logistic regression indicated that BaPWV, BMI and age were independent risk factors of CI-PEFs. ConclusionBaPWV has potential screening value for early CI-PEF, and the faster baPWV, higher BMI and elderly age are the risk factors of CI-PEF.
