支气管扩张患者BMI与肺功能及疾病严重程度的相关性

摘要:
目的 探讨支气管扩张患者BMI与肺功能及疾病严重程度的相关性。方法将85例支气管扩张住院患者分为低BMI组(BMI<18.5kg/m2,n=14)、正常BMI组(18.5kg/m2≤BMI≤25kg/m2,n=47)和高BMI组(BMI>25kg/m2,n=24)。比较3组患者血气分析结果、CRP水平、降钙素原(procalcitionin,PCT)及常规肺功能检测结果;分析患者BMI与FVC、IVC、DLCO的相关性。结果低BMI组的体质量、PaO2、DLCO、FVC、FEV1%pred、IRV低于正常BMI组(P<0.05)。低BMI组的体质量、PaO2、DLCO、FVC、IRV、IVC低于高BMI组(P<0.05)。低BMI组的PCT高于正常BMI组和高BMI组(P<0.05)。将85例支气管扩张患者的BMI与FVC、IVC、DLCO进行Pearson相关分析,并进行显著性检验,提示BMI与FVC、IVC、DLCO呈正相关。结论低BMI组的DLCO和FEV1%pred减低更显著,炎症反应更严重。

Abstract:
Objective To investigate the correlation of body mass index (BMI) with lung function and disease severity in patients with bronchiectasis. MethodsEighty five patients with brochiectasis were divided into three groups according their BMI: normal BMI group (18.5kg/m2≤BMI≤25kg/m2, n=47), high BMI group (BMI>25kg/m2, n=24)and low BMI group (BMI<18.5kg/m2, n=14). The levels of blood glucose, C-reacting protein (CRP), procalcitionin (PCT) and pulmonary function test were compared among three groups. The correlation of BMI with FVC, IVC and DLCO was analyzed. ResultsThe body mass, PaO2, DLCO, FVC, FEV1% pred and IRV of the low BMI group were lower than those of the normal BMI group. The body mass, PaO2, DLCO, FVC, IRV, IVC of the low BMI group were lower than those of high BMI group. PCTs levels in the low BMI group were higher than those in the normal BMI group and the high BMI group. Pearson correlation analysis showed that BMI positively correlated with FVC, IVC and DLCO. ConclusionThe lung function in brochiectasis patients with low BMI is significantly lower, and the inflammation is more severe and more active.
