目的 探讨尿沉渣RUNX3基因甲基化检测联合B超检查在膀胱癌随访中的作用。方法2014年9月至2015年2月,对50例初发膀胱癌患者行经尿道膀胱肿瘤电切术,术后病理证实为非肌层浸润性尿路上皮癌。术后2年内,所有患者每3个月随访复查1次,行膀胱B超检查、尿沉渣RUNX3基因甲基化检测、尿脱落细胞学检查及膀胱镜检查。结果50例患者膀胱镜检查并活检发现肿瘤复发28人次,联合膀胱B超检查、尿沉渣RUNX3基因甲基化检测的联合检测法诊断膀胱癌复发的灵敏度92.86%,特异度91.62%,Youden指数0.8448,阴性预测值高达99.35%。结论膀胱B超检查联合尿沉渣RUNX3基因甲基化检测膀胱癌复发,灵敏度高、特异性强,阴性预测值高。 |

Objective To evaluate of combination of RUNX3 gene methylation detection in urine sediment with transabdominal ultrasonography for the follow up of non-muscle-invasive bladder cancer. MethodsFifty patients with primary bladder cancer, who were underwent transurethral resection (TUR) from September 2014 to February 2015 and non-muscle-invasive urothelial carcinoma was confirmed by postoperative pathology, were enrolled in this study. The patients were followed up every 3 months for 2 years after TUR. Cystoscopy, transabdominal ultrasonography and urine cytology were performed; and methylation of RUNX3 was detected in urine sediment during the follow-up. ResultsAmong 50 cases, tumor recurrence was found in 28. For parallel test of detection of RUNX3 methylation in urine sediment with transabdominal ultrasonography, the sensitivity, specificity, Youden index and negative predictive value were 92.86%, 91.62%, 0.8448 and 99.35%, respectively. ConclusionCombination of RUNX3 methylation detection in urine sediment with transabdominal ultrasonography is convenient and noninvasive in the follow up study of non-muscle-invasive bladder cancer, with high sensitivity and specificity. The negative result may indicate the extremely low possibility of recurrence of bladder cancer, which might reduce the times of cystoscopy. |
