增生性瘢痕胶原结节中成纤维细胞生物学行为的研究

摘要:
目的研究增生性瘢痕胶原结节中成纤维细胞(fibroblast, Fb)生物学行为及其意义。方法 收集瘢痕切除植皮手术中切除的瘢痕组织及剩余的正常皮肤标本,H-E染色观察正常皮肤与瘢痕组织胶原结节及非结节区域的组织形态学差异,免疫组织化学法检测正常皮肤与瘢痕组织中Ki67及α-平滑肌肌动蛋白(α-smooth muscle actin,α-SMA)的表达及分布情况。结果 与正常皮肤及增生性瘢痕非胶原结节区域相比,增生性瘢痕胶原结节中可见大量Fb,Ki67及α-SMA阳性表达的Fb亦主要分布于胶原结节区域。结论 胶原结节处Fb增殖活跃,并大量分化为肌成纤维细胞(myofibroblast, MFB)。MFB的大量存在提示局部组织处于高张力水平,后者可能是导致Fb功能活跃、大量存在从而形成胶原结节的重要机制之一。

Abstract:
Objective To investigate the biological behavior of fibroblasts (Fbs) in collagen nodules of hypertrophic scar. Methods Scar and normal skin specimens were collected from patients with scar excision and skin grafting operation. The histological differences between normal skin and collagen nodules and non-nodular area were observed by light microscopy with H-E staining. The expression and distribution of Ki67 and alpha-smooth muscle actin (α-SMA) in normal skin and scar tissue were detected by immunohistochemical analysis. Results Compared with the normal skin and non-nodular area in hypertrophic scar, collagen nodules showed a large amount of Fb, and the positive expression of Ki67 and α-SMA was distributed mainly in Fbs located in collagen nodules. ConclusionFibroblasts in collagen nodules are proliferated actively, and differentiated to myofibroblasts, leading to the high tension of local tissue, which may be an important mechanism of collagen nodules formation.
