上海市高行社区老年人群糖尿病前期的患病率及其危险因素的调查分析

摘要:
目的 调查上海老年人群糖尿病以及糖尿病前期的患病率情况,并探索导致其高发的危险因素。方法 采用整群抽样以及回顾性调查分析,采用多元回归模型探讨导致空腹血糖升高的危险因素。结果 共有3950名研究对象参与了本次研究,其中男性1745名,女性2205名。偏相关分析提示: 空腹血糖水平与BMI呈现出中等程度的相关性(偏相关系数为0.20,P<0.05);与SBP、DBP、AST、hs-CRP呈现弱相关性(P<0.05);与HDL、UA及Cr存在负相关性。Logistic回归模型提示: 老年、肥胖、较高的收缩压、较低的HDL水平及较低的Cr水平是导致糖尿病前期发生的高危因素,其对应的OR值分别是1.01、1.66、1.01、0.47和0.99。结论 上海高行社区老年人群中达到糖尿病前期状态的人群患病率为23.50%。老年、肥胖、较高的收缩压及较低的HDL水平是导致糖尿病前期发生的高危因素。

Abstract:
Objective To estimate the prevalence of diabetes and pre-diabetes among elderly population in Shanghai Gaohang community and to explore the associated risk factors. Methods The cluster sampling and retrospective analysis were conducted in research design. Multiple regression models were used to explore the risk factors for diabetes and pre-diabetes. Results A total of 3950 subjects participated in this study, including 1745 males and 2205 females. The partial correlation analysis indicated that fasting blood glucose level had a moderate correlation with BMI with a partial correlation coefficient of 0.20 (P<0.05); and it showed a weak correlation with SBP, DBP, AST, hs-CRP (P<0.05); and negative correlation with LDL, UA, Cr. Logistic regression models suggested that older age, obesity, higher SBP, lower HDL, and lower Cr levels were risk factors related to pre-diabetes with OR values of 1.01,1.66,1.01,0.47 and 0.99, respectively. ConclusionThe prevalence of pre-diabetes and diabetes was 23.50% in the elderly population of Shanghai Gaohang community. Older age, obesity, higher SBP, and lower HDL are high risk factors leading to pre-diabetes.
