HIFU联合GnRH-a治疗子宫腺肌病剂量学研究

摘要:
目的 探讨高强度聚焦超声(high intensity focused ultrasound, HIFU)联合促性腺释放激素激动剂(gonadotropin releasing hormone agonist, GnRH-a)治疗子宫腺肌病剂量学研究。方法 选择在同济大学附属第一妇幼保健院接受治疗的子宫腺肌病患者66例,随机分为A、B、C三组,分别应用HIFU、HIFU术前1~2针GnRH-a预处理、HIFU术前3针GnRH-a预处理,评价病灶情况,记录HIFU辐照时间及辐照能量,以能效因子(EEF)为评价指标,分析GnRH-a对HIFU治疗子宫腺肌病的所需能量。结果 A组EEF为(37.37±75.09)J/mm3;B组EEF为(12.95±21.89)J/mm3,C组EEF为(1.54±0.47)J/mm3。C组分别与A组、B组比较,差异有统计学意义(P<0.05)。结论 3针GnRH-a预处理能明显降低HIFU治疗子宫腺肌病所需的能量,从而缩短HIFU治疗时间,提高治疗效率,其联合应用具有良好的临床价值。

Abstract:
Objective To investigate the optimal dose of gonadotropin releasing hormone agonist (GnRH-a) in combination of high intensity focused ultrasound (HIFU) for treatment of adenomyosis. Methods Sixty six patients with adenomyosis admitted in Shanghai First Maternity and Infant Hospital were randomly divided into three groups with 22 cases in each. Patients receive 1 (group A), 2 (group B) or 3 (group C) injections of GnRH-a before HIFU treatment. The size of lesion, the HIFU irradiation time, the radiation energy and energy efficiency factor (EEF) were documented. The effect of GnRH-a doses before HIFU treatment on adenomyosis was analyzed. Results The EEF in groups A, B and C were (37.37±75.09) , (12.95±21.89) and (1.54±0.47) J/mm3, respectively, that of group C was significantly lower than those of groups A and B(P<0.05). Conclusion Three injections of GnRH-a before HIFU can significantly reduce the energy of HIFU, shorten the treatment time and improve the efficiency in treatment of adenomyosis.
