可降解JDBM支架与镍钛合金支架植入兔腹主动脉术后的对比研究

摘要:
目的对比生物可降解镁合金支架JDBM与镍钛合金支架置入健康新西兰大白兔腹主动脉后的安全性和有效性。方法采用生物可降解镁合金血管支架(JDBM组)和镍钛合金支架(BMS组)植入24只新西兰大白兔的腹主动脉内,分别在植入后第14天、30天和60天行DSA、血管弹性和血液学检查。结果JDBM组和BMS组在各观察终点均未出现动物死亡,无血管急性血栓、夹层、穿孔、支架移位或支架内狭窄等情况。在术后14d,JDBM组较BMS组血小板计数[(451.3±47.4)×109/L vs (347.8±62.5)×109/L,P<0.05]和血清NO含量[(109.7±19.5)μmol/L vs (82.2±23.2)μmol/L,P<0.05]有一过性升高,术后30d后恢复至术前水平。JDBM组支架处的血管弹性从术后即时(D0)至术后60d(D60)呈好转趋势,而BMS组支架处血管弹性始终劣于JDBM组。结论可降解镁合金血管支架JDBM在围手术期安全有效,具有良好的组织相容性,未增加支架内血栓形成和再狭窄的发生,并能改善植入段血管弹性。

Abstract:
ObjectiveTo compare the efficacy and safety of bare biodegradable magnesium alloy JDBM stent with bare nitinol stent implanted in abdominal aorta of rabbits. MethodsBare biodegradable magnesium alloy JDBM stents(JDBM stent, n=12) or bare nitinol stents(BMS group, n=12) were implanted in abdominal aorta of New Zealand white rabbits. At 14th, 30th and 60th day after implantation, abdominal aortic angiography, artery elasticity and blood tests were performed. ResultsAngiography showed that all stents were patent with normal flow. No death, perforation, dissection, stent migration, in-stent thrombosis or restenosis were observed in either group. The level of blood platelets[(451.3±47.4)×109/L)] and serum NO[(109.7±19.5)μmol] were significant higher in JDBM group in comparison with BMS group[(347.8±62.5)×109/L, 82.2±23.2 μmol/L)], then returned to baseline at d30. The elasticity of arteries where implanted JDBM stents was recovered at d60 after operation, the stiffness parameter β in JDBM group was consistently better than that in BMS group. ConclusionThe implantation of biodegradable magnesium alloy JDBM vascular stents in abdominal aorta of rabbits is effective and safe. The JDBM stent shows favorable biocompatibility in vivo and superiority in artery elasticity compared with nitinol stent.
