FDS联合TCT检查对乳头溢液诊治的意义

摘要:
目的 探讨纤维乳管内视镜(fiberoptic ductoscopy, FDS)检查冲洗液液基薄层细胞学检查(thinprep cytologic test, TCT)对乳头溢液的诊治价值。方法 对186例已行B超检查的乳头溢液患者进行FDS和冲洗液TCT检查,142例行手术治疗,将各种检查与组织细胞病理结果进行对照分析。结果 FDS的确诊率为81.0%,TCT为78.9%,两者联合为95.1%,超声为62.0%,FDS联合TCT较其他单种检查准确性差异显著(P <0.01)。结论 纤维乳管内视镜联合液基薄层细胞学检查与组织病理学诊断符合率高,对病理性乳头溢液的早期诊治有确实的临床意义。

Abstract:
ObjectiveTo evaluate fiberoptic ductoscopy(FDS) combined with thinprep cytologic test(TCT) in diagnosis of nipple discharge. Methods FDS, TCT examination and ultrasonography were performed in 186 patients with nipple discharge,among whom 142 received surgical treatment. With histopathological examination as gold standard, the diagnostic value of each method was analyzed. Results The diagnosis accuracy rate of FDS, TCT and untralsonography alone was 81.0%, 78.9% and 62%, respectively; while that of FDS and TCT combination was 95.1%(P<0.01). Conclusion FDS combined with TCT has high diagnostic accuracy,and it may be of value in early diagnosis of nipple discharge.
