目的 探讨磁共振结肠成像(magnetic resonance colonography, MRC)在评估活动性溃疡性结肠炎严重程度中的价值。方法 收集经临床、肠镜及病理证实的活动性溃疡性结肠炎患者23例,根据改良Truelove-Witt's标准将患者分为轻度、中度、重度三组,比较不同组间MRC表现差异。MRC主要评价指标: 黏膜强化、肠壁增厚、肠壁水肿、周围增大淋巴结、梳齿征、深溃疡、病变段DWI高信号。结果 23例活动性溃疡性结肠炎患者中,轻度组4例,中度组8例,重度组11例。肠管周围淋巴结增大在轻度组与重度组间有显著性差异(P=0.009);肠壁水肿分层和梳齿征在轻度组与重度组间有显著性差异(P=0.001,P=0.004),在中度组与重度间也有显著性差异(P=0.018,P=0.041);黏膜强化、肠壁增厚及DWI高信号在三组间无显著性差异(P=0.16);深溃疡仅出现在重度组。结论 磁共振结肠成像(MRC)在评估活动性溃疡性结肠炎严重程度中有一定价值。 |

Objective To assess magnetic resonance colonography(MRC) in evaluation of disease severity in patients with active ulcerative colitis(UC). Methods Twenty-three patients with active UC confirmed by clinical manifestation, colonoscopy and pathology underwent MRC examination. According to the modified Truelove-Witt's criteria, the patients were classified as mild group(n=4), moderate group(n=8) and severe group(n=11). MRC parameters:mucosa hyperenhancement,wall thickness,mural edema, enlarged mesenteric lymph nodes, comb sign, deep ulcers, diffusion-weighted imaging(DWI) hyperintensity of the lesion segment were evaluated and compared among 3 groups. Results There was significant difference in enlarged mesenteric lymph nodes between mild and severe groups(P=0.009). There were significant differences in mural edema and comb sign between mild and severe groups(P =0.001,P=0.004), and between moderate and severe groups(P=0.018,P=0.041). However, there were no significant differences in mucosa hyperenhancement, bowel wall thickening and DWI hyperintensity of the lesion segment among three groups(P=0.16); the deep ulceration only seen in severe group. Conclusion MRC has a certain value for the assessment of disease severity in patients with active ulcerative colitis. |
