超声在胎儿颅内出血诊断中的应用

摘要:
目的 探讨超声诊断胎儿颅内出血的应用价值。方法 回顾性分析胎儿颅内出血的超声图像特点,所有病例经核磁共振(magnetic resonance imaging, MRI)进行确诊。结果 依据胎儿颅内出血分度标准,Ⅰ度为出血局限于室管膜下尾状核的生发基层;Ⅱ度出血到达脑室;Ⅲ度指出现继发脑室扩张;Ⅳ度为脑实质出血。共发现胎儿颅内出血Ⅰ度0例,Ⅱ度14例,Ⅲ度4例,Ⅳ度4例。颅内出血表现为高回声6例,混合回声7例,无回声9例。所有病例经MRI检测核实,均符合颅内出血诊断。结论 超声检查准确率高,较MRI更经济,适合产前筛查。通过超声检查对颅内出血进行分级,可对临床医师提供产前咨询和治疗提供诊断依据。

Abstract:
Objective To investigate the value of ultrasonography in diagnosis of fetal intracranial hemorrhage. Methods The ultrasonographic findings of fetuses with intracranial hemorrhage were retrospectively reviewed. All the cases were demonstrated by MRI. Results According to the grading standards fetal intracranial hemorrhage was classified as follows, grade Ⅰ:hemorrhagic foci confined in togerminal base of endyma caudate nucleus; grade Ⅱ:hemorrhagic foci extended to encephalocoele; grade Ⅲ:secondary ventricular dilatation; grade Ⅳ:cerebral hemorrhage. There were no case with grade Ⅰ intracranial hemorrhage, 14 in grade Ⅱ, 4 in grade Ⅲ and 4 in grade Ⅳ. Six cases were showed high echo, 7 were showed mix echo and 9 cases were echoless. ConclusionUltrasonography is highly effective in detecting the fetal intracranial hemorrhage. The ultrasound classification can be helpful in early clinical intervention for pregnant and prenatal consultation.
