集体治疗对大龄脑瘫儿童运动功能和功能独立性的影响

摘要:
目的 评价集体治疗对大龄脑瘫儿童运动功能和功能独立性的影响。方法 选取符合入组条件的60名大龄脑瘫患儿,随机分入常规治疗组30名和集体治疗组。常规治疗主要为手法治疗和运动训练为主,进行个体辅导训练;集体治疗组主要以游戏或互动性活动为主,进行集体活动训练。时间约80min,每周2次,持续24周。所有患儿在治疗前、治疗3个月和治疗6个月后采用粗大功能评定量表(GMFM)、精细运动功能评定量表(FMFM)和儿童功能独立性评定量表(weeFIM)评定。结果 治疗前两组间GMFM评分、FMFM评分、FIM评分差异均无统计学意义。治疗3个月和6个月后,两组间三项评分差异虽无统计学意义(P>0.05),但集体治疗组GMFM评分、weeFIM评分较常规治疗组均有增高趋势。组内比较,治疗3个月和6个月后,集体治疗组GMFM评分、weeFIM评分均较入组时提高,差异均有统计学意义(P<0.05),常规治疗组的GMFM评分、weeFIM评分和两组FMFM评分虽也有增加,但差异无统计学意义。结论 对大龄脑瘫患儿采取不同的内容和形式的集体治疗,可能有助于提高脑瘫患儿的运动功能和独立性。

Abstract:
Objective To assess the effect of group-based therapy on the motor function and ability of independence in elder children with cerebral palsy.Methods Sixty elder children with cerebral palsy were assigned into group-based therapy(GT) and routine treatment(RT) groups randomly with 30 patients in each. The children in the RT group were treated “one on one” with manual therapy and motor-training therapy, while those in GT group were treated in forms of games and interactive activities in group. The treatment lasted for 24 weeks, twice a week with approximately 80 min for each time. All the children were assessed with GMFM, FMFM and weeFIM before the treatment, immediately after 3-month treatment and 6-month treatment. ResultsThere were no statistical differences in GMFM, FMFM and weeFIM scores between groups before the treatment. After 3- and 6-month treatment, GMFM and weeFIM scores in GT group tend to be higher than that in the RT group, but no statistical differences were found(P>0.05). After 3- and 6-month treatment, the GMFM and weeFIM scores in GT group were statistically higher than that before treatment(P<0.05), while there were no significant differences in RT group. Conclusion Group therapy may improve the motor function and ability of independence in elder children with cerebral palsy.
