大孔径螺旋CT低剂量扫描在乳腺癌放疗模拟定位中的应用

摘要:
目的 探讨大孔径20排螺旋CT低剂量扫描在乳腺癌放疗模拟定位中的应用。方法 采用大孔径20排螺旋CT模拟定位60例乳腺癌根治术后放疗患者,随机分为两组: 30例行低剂量扫描,30例行常规剂量扫描。30例常规剂量扫描者,在气管隆突平面再采用低剂量扫描3层。由2名有经验的放射科、放疗科主治医师分别对两组不同剂量的图像质量采用双盲法进行评分(5分法)并进行对比分析。同时分别测量常规剂量组两种剂量下同一层面肺组织的CT值,并进行统计学分析。结果 大孔径20排螺旋CT在放疗模拟定位中的应用避免了因患者体位受限而造成的误差。低剂量扫描与常规剂量扫描在图像质量评估上差异无统计学意义;在肺组织的CT值的测量上无统计学意义。结论 大孔径20排螺旋CT的应用,使放疗模拟定位更加准确,为精确计划和治疗提供了保证。低剂量扫描使放疗模拟定位辐射剂量降低,在保证质量的同时有效地保护了患者,具有较高的临床应用价值。

Abstract:
Objective To evaluate the application of large aperture 20-row spiral CT with low dose scanning in radiotherapy simulation positioning for the breast cancer patients. MethodsSixty patients with breast cancer after radical mastectomy undergoing radiotherapy simulation positioning with large aperture 20 row spiral CT were divided randomly into two groups according to the dosage of scanning radiation: 30 patients in regular dosage group and 30 patients in low dosage group.The low dosage group was performed with low-dose protocol scanning. Three slices with the identical low-dose protocol scanning were also performed at the level of tracheal carina area in regular dosage group after regular dosage radiation scanning. The scan images of both groups were evaluated and analyzed by an experienced radiologist and a radiation therapist independently with double blindness method. The image quality was scales into 5 grades. The CT densities of lung under regular protocol and low-dose protocol scanning were also measured and analyzed in regular dosage group. Results There was no statistical difference image quality between two groups. There was also no statistical difference in the CT densities of lung of the same patient between scans with two dosage protocols. Conclusion The application of large aperture 20 row spiral CT in radiotherapy simulation positioning improves the accuracy of radiotherapy positioning for breast cancer patients, which also can reduce radiation dose without effect on image quality.
