脑电图在脑脓肿患者的诊断和预后判断中的价值

摘要:
目的 对48例经临床和医技检查证实的脑脓肿病人回顾性分析了脑电图头颅核磁共振资料,研究脑电图在脑脓肿的早期诊断和判断预后的价值。方法 48例患者经临床及医技检查,确诊为脑脓肿,保守治疗15例,微创穿刺引流脓肿12例,手术切除脓肿21例,脑电图分别在脑脓肿患者治疗前,保守治疗和手术后10~95d(平均45d)进行,日本光电16导脑电图仪,参照国际10/20电极安置法系统放置头皮电极,FPz接地线,分别进行参考导联的单极、纵向和环形的双级导联描记常规脑电图。结果 48例脑电图在脑脓肿疾病早期就表现特异性的不正常,经过保守和手术治疗后脑电图显著好转,提示由于脑脓肿早期由于脑水肿,造成正常神经细胞的活动障碍,结果出现弥漫性的异常慢波,经过治疗后,临床症状改善消失,脑电图也随之改善。结论 脑电图检查有助于发现脑脓肿早期引起的大脑皮层功能改变,辅助头颅核磁共振检查可以早期明确诊断;比较脑脓肿治疗前后的脑电图差异,可以对脑脓肿的诊断、治疗、判断病情变化及预后,脑电图检查方便简易具有直观的价值。

Application of electroencephalography in diagnosis and prognosis of patients with brain abscess

Abstract:
Objective To assess the application of electroencephalogramphy(EEG) in diagnosis and prognosis of brain abscess. Methods Forty eight patients with clinically diagnosed brain abscess were included in the study. Among all patients 15 received conservation treatment, 12 underwent minimally-invasive puncture and drainage for abscess, and 21 had surgical excision of abscess. EEG was performed before and 10-95d (mean 45d) after treatment. A photoelectric 16-lead electroencephalography was applied, and the electrodes were placed according to the international 10/20 system. The routine EEGs of first reference lead uni-polar, bipolar and annular lead tracings were performed. Results The EEGs displayed specific abnormalities in early stage of brain abscess. EEGs were significantly improved after effective conservative treatment or operation, and the abnormal diffuse slow wave disappeared. Conclusion EEG examination is of value in early diagnosis and prognosis of brain abscess.
