目的 讨论重度及特重度烧伤早期血胃肠激素的测定及其意义。方法 选取64例重度烧伤患者为研究对象,通过分时段采血,测定胃泌素、胃动素和β-内啡肽。结果 重度及特重度烧伤患者,早期胃泌素的分泌量大幅下降,伤后8h内急剧下降,并在9~24h开始回升,随后呈现出小幅下降,最终维持在低于正常的水平;胃动素自伤后2h开始下降,然后在患者休克期内保持较低水平,并在伤后9h左右到达到最低水平,进入水肿回吸期开始逐渐提升,但仍低于正常水平;β内啡肽在伤后便会出现明显升高,伤后8h达到最高峰。当烧伤面积<70%时,随着烧伤面积的不断增大,胃泌素、胃动素的降低明显,而β内啡肽则升高。结论 重度烧伤早期,患者的血清胃肠激素可以在一定程度上反映患者的预后。 |

Abstract: |
Objective To evaluate the measurement of blood gastrointestinal hormones in patients with early stage of severe and very severe burns. MethodsSixty four patients with severe and very severe burns were included in this study. Blood samples were collected at different time points and serum gastrin, motilin and β-endorphin levels were measured with radioimmunoassay. ResultsSerum gastrin levels were dropped in early phase, then raised in 8h and at the peak level in 9~24h, followed by slightly declining and remaining in a relatively stable level. Serum motilin levels were declined in 2h after burn and maintained a low level during shock period, bottomed at 9h; then rose in edema suction stage, but failed to return the normal level. β-endorphin levels were increased significantly after burn, reached to a peak within 8h. The decline of gastrin, motilin and elevation of β-endorphin levels were correlated with the burn area when it <70%. ConclusionAt the early stage of severe and very severe burns, serum gastrointestinal hormones levels may indicate the prognosis of patients to a certain extent. |
