卵泡微环境改变对多囊卵巢综合征患者体外受精-胚胎移植的影响

摘要:
目的研究在体外授精(in vitro fertilization,IVF)治疗过程中多囊卵巢综合征患者(polycystic ovarian syndrome,PCOS)与非多囊卵巢综合征患者的对照组促排卵后内分泌的差别,胚胎质量和妊娠结局的差异,探讨内分泌情况以及卵泡微环境对PCOS患者在IVF治疗中卵泡发育以及妊娠结局的影响。方法将自2011年12月—2013年3月在上海市第一妇婴保健院生殖中心行IVF助孕的患者分成对照组和PCOS组。比较两组的一般情况,内分泌以及卵泡液中激素水平改变,以及胚胎质量和IVF妊娠结局等。结果两组一般情况无明显区别,PCOS的获卵率明显低于对照组,而受精数,卵裂数,妊娠率等无明显差别。PCOS组中卵泡液睾酮(T)水平高于对照组。结论 PCOS患者内分泌紊乱尤其是高水平的卵泡液睾酮会影响卵子发育成熟,降低成熟卵子的获得率,但是卵子及胚胎质量并未受到影响,临床在促排卵前应注重改善PCOS微环境,提高成熟卵子获得率。

Abstract:
Objective To investigate the effect of microenvironment of follicle fluid on IVF-ET results in patients with polycyclic ovarian syndrome( PCOS). Methods One hundred and thirty eight patients with PCOS and 86 infertility w omen without PCOS( controls) received in vitro fertility-embryo transfer( IVF-ET) from December 2011 to March 2013 were included in the study. The endocrine,embryo quality and IVF results were compared between two groups. Results There were no significant differences in general conditions between two groups. The oocyte harvest rate in PCOS patients was significantly low er than that of controls; the testosterone level in follicle fluid of PCOS patients was higher than that of controls. There were no significant differences in fertilization number and cleavage number between two groups. Conclusion The change of testosterone level in follicle fluid of PCOS patients may affect the development of oocyte but not the quality of embryo.
