64层容积CT低剂量对比剂肺动脉成像的临床研究

摘要:
目的探讨64层容积CT低剂量对比剂肺动脉成像的临床应用可行性。方法对临床怀疑肺动脉栓塞且无心肾功能异常的患者50例行64层容积CT肺动脉成像检查,根据对比剂使用剂量将其随机分为2组:常规剂量组注射对比剂剂量80 ml;低剂量组注射对比剂40 ml后随即注射生理盐水40 ml,注射速率都相同。两组患者CT扫描参数相同,均使用非离子对比剂碘海醇370 mgl/ml,注射速率为4 ml/s。由2位有经验的放射科主治医师分别对两组不同剂量对比剂成像的肺动脉图像,采用双盲法进行独立分析、评价。影像质量按5级评价,并测量肺动脉与相邻肺静脉内对比剂浓度的CT值差。结果两组肺动脉主干及主要分支均显示均良好,两组间肺动脉与相邻肺静脉内对比剂浓度的CT值差存在显著性差异。两组CTPA图像质量评价显示,常规剂量组5级图像8例,4级图像11例,3级图像6例,低剂量组5级图像7例,4级图像13例,3级图像5例,两组均未见1级、2级图像。两组间图像质量没有显著性差异,都可达到诊断要求。结论64层容积CT肺动脉成像采用40 ml的低剂量对比剂可使怀疑肺栓塞患者的肺动脉各级分支显影,对图像质量没有影响,能够达到诊断要求,并降低对比剂毒性,具有较高的临床应用价值。

Abstract:
Objective To evaluate the clinical application of 64-slices volume CT pulmonary angiography with low dose contrast media.Methods Fifty patients with suspected pulmonary embolism and normal heart functions scheduled for 64-slices volume CT pulmonary angiography were randomly assigned to use regular dose of contrast media(80 ml) or low dose(40 ml),with 25 patients in each group.The identical CT scanning parameter was used in both groups,and the non-ion contrast agent iohexol with the concentration of 370 mgl/ml was injected with the flow rate of 4 ml/s.The images were evaluated and analyzed by two experienced radiologists independently.The image quality was evaluated with 5-grade standard,the CT density subtraction of pulmonary artery and the adjacent pulmonary vein were also measured.Results All pulmonary artery trunk and the major branches in both groups were well delineated.There was significant difference in CT density subtraction of pulmonary artery and the adjacent pulmonary vein between the two groups.The image quality scores of regular dosage group were rated 5 in 8 cases,4 in 11,3 in 6,and those of low dosage group were rated 5 in 7 cases,4 in 13,3 in 5.There was no statistical difference in image quality between two groups.Conclusion Low dose contrast media can be used in 64-slices volume CT pulmonary angiography to diagnose pulmonary embolism accurately with reduced toxicity of contrast media,which is of high value in clinical application.
