| 目的探讨子宫动脉栓塞与刮宫术在治疗子宫疤痕妊娠中的应用价值。方法回顾性分析2010年6月至2012年6月我院收治的15例剖宫产术后子宫疤痕妊娠（CSP）患者的临床资料。所有患者术前均经临床及超声检查明确诊断，在刮宫术前预防性于双侧子宫动脉置入明胶海绵颗粒（700～1000μm）进行栓塞，术后行刮宫术。结果15例患者于子宫动脉栓塞术后5d内行刮宫术，术中出血少（〈50ml），全部保留子宫，无栓塞后并发症发生。刮宫术后继续住院时间1-2d（平均1．5d），出院后电话随访半年，15例患者恢复月经。结论CSP患者预防性使用双侧子宫动脉栓塞术，可有效降低子宫大出血的风险，并能保留子宫及生育功能，此法安全有效，无严重并发症发生，对CSP可作为首选的治疗方法。 |

| Objective To evaluate the application of uterine artery embolization and curettage in treatment of cesarean scar pregnancy. Methods Clinical data of 15 women with cesarean scar pregnancy treated in Tongji hospital from June 2010 to June 2012 were retrospectively analyzed. All patients were diagnosed according to clinical sign and color Doppler ultrasonoraphy, and treated with curettage. Before curettage bi-lateral uterine arteries were embolization by Seldinger technique with Gelfoam particles（700 - 1 000 μm）. Results The curettage was performed within 5 d after arterial embolization and patients had bleeding 〈 50 ml during curettage. The uterus were retained successfully without complications after embolization in all 15 patients. The average of length ofhospital stay was 1.5 d （1 -2 d ）after curettage. The follow-up at 6 months after discharge showed that normal menstruation was recovered in all patients. Conclusion Uterine artery embolization before curettage is effective and safe in treatment of cesarean scar pregnancy, which can retain the uterus and reproductive function without serious complications. |
