| 目的比较72例老年ASON Ⅲ型桡骨小头粉碎性骨折内固定与桡骨小头置换治疗的中短期疗效。方法2001年1月至2012年6月，72例老年桡骨小头MasonⅢ型骨折患者采用切开复位内固定术和人工桡骨小头置换术治疗，其中固定组35例，置换组37例，并进行10~15.6个月随访。效果评估根据Broderg和Morrey评分标准及肘关节痛觉视觉模拟（VAS）进行评分。结果Broberg和Morrey评分，内固定组中优24例，良3例，可6例，差2例，优异率达68.6％，置换组中优29例，良6例，可1例，差1例，优异率高78.4％。内固定组平均肘关节痛觉VAS评分为16.7分，置换组22.5分，两组P＜0.05，有明显统计学差异。结论相比切开复位内固定术而言，桡骨小头置换治疗MASONⅢ型桡骨小头粉碎性骨折，疗效较好。 |

| Objective To compare open reduction and internal fixation （ORIF） with radial head prosthesis replacement in treatment of radial head fractures of Mason type Ⅲ in 72 elderly patients. Methods Seventy two elderly patient with Mason type Ⅲradial head fracture were treated during January 2001 and June 2012, among whom 37 cases received radial prosthesis and 35 cases were treated with ORIF. All patients were followed-up for 10 to 15.6 months. Results According to elbow functional evaluation criteria by Broberg and Morrey, excellent results were found in 29 cases, good in 7 and fair in 1 for replacement group; meanwhile, there were excellent results in 24 cases, good in 9, and fair in 2 for ORIF group; the rates of good or excellent were 78.4% and 68.6% for prosthesis replacement patients and for ORIF patients, respectively （P＜0.05 ）. The Visual Analogue Scores （VAS） for replacement and ORIF groups were 22.5 and 16.7, respectively （P＜0.05 ）. Conclusion The radial head prosthesis replacement is better than ORIF in treatment of Mason type Ⅲ radial head fracture in elderly patients. |
