重症脑卒中患者早期肠内营养治疗的临床观察

摘要:
目的探讨重症脑卒中患者肠内营养支持治疗的临床意义。方法伴意识障碍的脑卒中患者64例,随机分为治疗组(34例)、对照组(30例),两组病例的基本情况和一般治疗差异无统计学意义(P>0.05)。入院72 h内,治疗组通过鼻胃管给予个性化的高能营养液104.6~146.4 kJ/(kg.d)治疗;对照组给予传统的鼻饲法。观察并比较两组营养状况和感染并发症的发生情况。结果治疗后治疗组的血清白蛋白、前白蛋白、外周血淋巴细胞计数治疗组明显高于对照组(P<0.05)。而且治疗后肺部感染、电解质紊乱、上消化道出血的发生率对照组明显高于治疗组(P<0.05)。结论重症脑卒中患者早期肠内营养支持治疗对改善患者一般情况、减少并发症、加快康复有重要意义。

Abstract:
Objective To evaluate the administration of early enteral nutrition support in patients with severe cerebral stroke. Methods Sixty four stroke patients with disturbance of consciousness in neurological ICU were divided into treatment group (n = 34) and control group (n = 30) ; the general conditions and basic treatment of two groups were compared (P >0.05) . Within 72 h of onset, patients in treatment group received enteral nutritional support with individualized high-energy nutritional solution 104.6 - 146. 4 kJ/ (kg ? d) through nasogastric tube; while patients in control group received conventional nasal feeding. The nutritional status and clinical complications were observed and compared between two groups. Results The serum albumin, pre-albumin, lymphocyte count were higher, and the incidence of lung infection, electrolyte imbalance, upper gastrointestinal bleeding in treatment group were lower than those in control group (all P <0.05) . Conclusion Early enteral nutrition support in patients with severe stroke can improve the general condition, reduce complications and accelerate recovery.
