| 目的探讨胆总管探查一期缝合的治疗效果及可行性。方法回顾分析2007年1月至2009年12月胆总管结石择期行胆总管探查一期缝合143例的临床资料（胆总管一期吻合组）,并与胆总管探查T管引流术（普通对照组200例）作一对比研究,对其手术适应证、手术方法及并发症进行总结。结果术后11例腹腔引流液含胆汁,10例未予特殊处理,1例行2次手术引流后痊愈。全组无手术死亡及残余结石,平均术后住院8.71 d。结论胆总管探查一期缝合胆管有其可行性,但必须以严格掌握手术适应证及精细的手术操作为前提。 |

| Abstract: |
| Objective To study the treatment effect and feasibility in primary closure of common bile duct following choledochotomy for common bile duct calculi.Methods One hundred and forty-three cases underwent common bile duct exploration with primary suture from January 2007 to December 2009 and 200 cases with exploration of common bile duct as control were analyzed retrospectively. Operative indication,methods and its complication were compared.Results Postoperative bile leakage was found in 11 patients and no reoperation was necessary expect one patient needed reoperation for drainage.No death and no retained stones in common bile duct were found and the mean hospitalization time was 8.71 days.Conclusion Common bile duct exploration with primary suture is feasible but should select operative indication strictly and operatie precisely. |
