| 目的比较食管引流型喉罩（PLMA）和标准型喉罩（SLMA）在妇科腹腔镜麻醉中的应用。方法 80例ASAⅠ～Ⅱ级、拟在全麻下行妇科腹腔镜手术的患者,随机分成PLMA组和SLMA组,每组40例,麻醉后分别插入PLMA或SLMA。评价两种喉罩置入时间、机械通气漏气率、麻醉过程中心率（HR）、血压（BP）的变化以及通气过程中气道峰压（Pmax）、平均压（Pmean）和呼气末二氧化碳（PetCO2）的变化。记录麻醉期间呼吸并发症和咽喉部损伤情况。结果盲探插入时,SLMA组一次插入成功率较PLMA高,操作时间也短（P〈0.05）,但PLMA组通气过程中漏气发生率较少（P〈0.05）。两组患者对插入操作的反应较小,程度相似,都有可能引起咽喉部黏膜少许出血。结论盲探插入时SLMA组一次插入的成功率较PLMA高,PLMA组通气中漏气率低。妇科腹腔镜手术时,PLMA通气满意率较高,更安全。 |

| Abstract: |
| Objective To compare the applications of ProSeal laryngeal mask airway（PLMA） with standard laryngeal mask airway（SLMA） in gynecological patients undergoing laparoscopic operation.Method Eighty gynecological patients with ASA Ⅰ-Ⅱ undergoing laparoscopic operation under general anesthesia were randomly allocated to either PLMA group or SLMA group.Insertion manipulation,the changes of HR,BP,the peak airway pressure,mean airway pressure as well as PetCO2 were assessed during anesthesia.At the same time respiratory complications and throat injury were recorded.Results Success rate was significantly higher in SLMA group than that in PLMA group by blind insertion（P0.05）.The insertion time was significantly shorter in SLMA group than that in PLMA group（P0.05）.However the unexpected event of the air leakage was significantly less in PLMA group（P0.05）.All the patients in the two groups had similar and mild hemodynamic response during the insertion process,and had certain throat mucous membrane injury.Conclusion Using SLMA has higher success rate but also increases air leakage rate.The ventilation is important for gynecological laparoscopic operation.Thus,it is more satisfactory and safer to use PLMA in gynecological patients undergoing laparoscopic operation. |
