人工股骨头置换治疗股骨转子间骨折的生物力学研究

摘要:
目的研究股骨转子间粉碎性骨折用股骨假体(标准柄)置换的生物力学稳定性。方法新鲜成年防腐股骨标本12例随机分成实验组及对照组。实验组模拟股骨转子间四部分骨折,钢丝捆扎固定大小转子及股骨矩,行股骨头置换,骨水泥固定;对照组模拟股骨颈骨折,行股骨头置换,骨水泥固定。万能试验机上行轴向加压和扭转加压,记录两组假体位移距离并行统计学检验。结果垂直加载100~1 000N,侧方加载100~800N,两组差异无统计学意义(P〉0.05)。结论股骨转子间骨折采用股骨标准柄置换的生物力学性能是可行的。

Abstract:
Objective To study the biomechanical stability of the communited intertrochanic fracture treated with cemented standard femoral stem prosthesis.Methods Twelve cadaveric femurs were selected in this study and divided into two groups randomly.The experimental group was simulated as communited intertrochanic fractures at four parts while the control group was simulated as the femoral neck fractures.Both groups were treated with cemented femoral prosthesis following standard hemiarthroplasty procedure and the four parts fractures in experimental group were fixed by wires additionally.The specimens were tested mechanically by universal testing machine through vertical and rotational loading.The displacement of the prosthesis while applying the loading were recorded and the differences of displacement magnitudes between two groups were analyzed.Results No significant differences of displacement were found between two groups either in vertical loading from 100N to 1000N or in rotational loading from 100N to 800N.Conclusion The use of femoral stem prosthesis is biomechanically acceptable in the treatment of intertrochanic fractures.
